. J NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-456 As a result of the inspection conducted on September 13 through October 24, 1987, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:

10 CFR 50, Appendix B, Criterion XVI, " Corrective Actions", is implemented by the Commonwealth Edison Quality Assurance Manual which states that a corrective action system will be used to assure that deviations and programmatic deficiencies which are adverse to quality and might affect the safe operation of a nuclear generating station are pr mptly identified and corrected.

Contrary to the above, the following events are examples of deviations or programmatic deficiencies for which short term corrective actions were not promptly or effectively implemented, resulting in recurrence while long term corrective actions were being developed.

1. On August 8, 1987, the Axial Flux Differential (AFD) Surveillance was not initiated per Technical Specification 4.2.1.1.a.(2) when the Unit 1 process computer was rebooted (thereby restoring the AFD monitor alarm).

This event recurred on September 8, 1987.

2. On January 2, 1987, a Unit 1 waste gas analysis for hydrogen and oxygen was not conducted per Technical Specification 3.3.10-1a, and on August 9, 1987, a reactor coolant analysis for iodine was not performed per Technical Specification 3/4.4.g, Table 4.4-1. A similar event took place on September 5, 1987, when an oxygen analysis was not performed on the waste gas system.

This is a Severity Level IV violation (Supplement I). (456/87035-01(DRP))

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
