SUBJECT: POINT BEACH INSPECTION REPORT 50-266/98011(DRP),

50-301/98011(DRP) {

Dear Mr. Patuiski:

On July 6,1998, the NRC completed an inspection at your Point Beach 1 and 2 reactor facilities.

The enclosed inspection report presents the results of that inspection.

During this 6-week inspection period, the Point Beach facility was operated safely. Following the

completion of the Unit 1 Cycle 24 refueling outage, the Unit i reactor was successfully restarted j

in a deliberate and controlled mar.ner. However, during the restart of the Unit i reactor, the

inspectors noted a deficiency with one of the reactor engineering procedures used to bring the

Unit 1 reactor critical, in that the procedure did not incorporate industry guidance for criticality

estimations when using boron dilution to achieve criticality. Although addressed during this

inspection period, this deficiency represented a non-conservative approach to reactivity ,

management and is considered another illustration of reactor engineering performance concerno )

discussed in previous inspection reports. j

Upon further review of the precedures associated with reactivity management, your staff

identified that procedure revisions had incorporated operating experience lessons leamed, but

the ' guidance was later removed. The deletion of procedural guidance during the procedure

change process, which has had detrimental effects, has been discussed in previous inspection

reports. The procedure change program methodology and the commitment management system

continue to be items warranting your attention.

During this inspection period, a Non-Cited Violation of NRC requirements was identified l

pertaining to the environmental qualification for the Unit 1 and Unit 2 containment hydrogen

monitors. This condition had apparently e>Lted for several years because of a failure to include

vendor environmental qualification specifications within routine maintenance procedures for the  ;

hydrogen monitors. While we have reviewed your corrective actions for the specific problem and j

found them acceptable, we request that in 30 days of the receipt of this letter you describe in

writing to the NRC the corrective actions that will be taken to ensure that requirements from I l

vendor manuals for all safety-related equipment are incorporated in plant procedures or j

otherwise controlled.
