SUBJECT: NRC INSPECTION REPORT 50-458/98-12 AND NOTICE OF VIOLATION
Dear Mr. McGaha:
From May 17 through June 27,1998, an inspection was conducted at your River Bend Station
reactor facility. The enclosed report presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the River Bend
Station facility was generally characterized by safety-conscious operations, sound engineering
and maintenance practices, and effective radiological controls. However, two violations were
,
identified during this inspection period. - The first violation involves an operator mistakenly pulling
control power fuses for the Division ll diesel generator output breaker, which momentarily
rendered the diesel generator inoperable. This is the third operator human performance error
!
we have observed during the past two inspection report periods that has had an impact on plant
operation, in your response to this violation, please discuss your actions to address the
increasing trend in Operations related human performance errors.

The second violation involves the failure to maintain emergency preparedness sirens operational
i
due to a problem with a computer software upgrade. Although this problem was identified during
a surveillance, post-installation testing of the software upgrade was inadequate and failed to
identify the problem, in response to this violation, please address your actions to ensure
thorough computer software testing.
The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detailin the enclosed report. Please note that you

are required to respond to this letter and should follow the instructions specified in the enclosed

Notice when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory
\\
'
requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure (s), and your response will be placed in the NRC Public Document Room (PDR).
