--PN-75-46 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by OIE staff as of 4:30 p.m. on date of this notification.

IT SHOULD BE SPECIFICALLY NOTED THAT THIS NOTIFICATION MAY CONTAIN INFORMATION THAT LATER MAY BE DETERMINED TO BE INACCURATE OR INCONSISTENT. AN OFFICE OF INSPECTION AND ENFORCEMENT NOTIFICATION OF AN INCIDENT OR OCCURRENCE (BLUE SHEET) SUBSEQUENTLY WILL BE ISSUED IF WARRANTED.

Subject:

Failure to Secure Licensed Byproduct Materials On July 1, 1975 a commercial carpet cleaning company employee, engaged in cleaning a rental trailer in Clinton, Oklahoma, found a lead pig which contained a sealed vial on which the label identified the contents as 20 mci Iodine-131 assayed on April 8, 1975. The trailer owner stated that the last renter was El Paso Natural Gas Co,, who returned the trailer

Clinton Civil Defense Director who notified the State of Oklahoma and agreed to turn the material over to'the state.

IE:IV contacted the licensee who is investigating and has agreed to arrange for the pickup of material from the State of Oklahoma. The Licensee believes the package may have been left in the trailer by one of their well logging service contractors.

Due to decay, radioactive content at this time is negligible. There has been no known publicity.

OIE (Region IV) received notification of this occurrence by telephone from the State of Oklahoma Department of Health at 11:00 a.m. on July 2, 1975.
