NRC INSPECTION REPORT 50-354/98-06 AND NOTICE OF VIOLATION

. On June 27,1998, the NRC completed an inspection at your Hope Creek reactor facility.-

The enclosed report presents the results of that inspection.

~ Throughout the six-week period covered by this inspection period, your conduct of

activities at the Hope Creek facility was generally characterized by safety-conscious

operations, sound engineering and maintenance practices, and careful radiological work

'

controls.-

l

Du ing the inspection, three violations of NRC requirements were identified. Two of the

violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding them are described in detail in the subject inspection report. The third

violation, which was common with the Salem facility, was cited separately in NRC .

Integrated inspection Report 50-272 & 50-311/98-05 by letter dated July 10,1998.

For the two violations cited in the enclosed Notice, the violations were self-revealing in

i

that a specific event or condition occurred as a consequence of the violation. In the first-

instance, chemistry personnel failed to perform emergency diesel generator (EDG) new fuel

oil particulate concentration analysis as required by procedures. A contaminated supply of

fuel oil was delivered and used, primarily for the two tanks associated with the 'B' EDG. A

separate, subsequent particulate con::entration analysis of the stored fuel oil resulted in

~ declaring the .'B' EDG inoperable. Your staff requested, and the NRC granted, a Notice of

.

Enforcement Discretion on May 22,1998, which exercised discretion not to enforce

compliance with EDG Technical Specification 3.8.1.1 on the basis that the action involved

' minimal or no safety impact and no adverse radiological impact to public health and safety.

Corrective actions for this issue were subsequently completed safely.

' A socorki violation was identified after the reactor core isolation cooling (RCIC) system

~ steam turbine rapidly oversped during post-maintenance testing during which the RCIC

pump and turbine were uncoupled. A modification that was performed during the Fall-

l/

Winter 1997 refueling outage was inadequately designed, reviewed and tested, and

/

resulted in defeating a feature of the steam admission stop valve control circuitry. This

. violation was of concern because it was the second recent example of a violation where a

' design related error that occurred during the last refueling outage challenged plant staff and

. equipment.

The third violation involved the failure to establish written procedures to collect drinking

water, fish, and invertebrate samples required by technical specifications and the Offsite

Dose Calculation Manual. This violation is common with the Salem facility and was

detailed in NRC Integrated Inspection Report 50-272 & 50-311/98-05. Your response to

this violation should be integrated with the response required for NRC Integrated Inspection

Report 50-272 & 50-311/98-05.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter

and its enclosures will be placed in the NRC Public Document Room (PDR).
