SPECIAL REPORT 87-19 DESCRIPTION OF EVENT On October 25, 1987, with unit 1 in mode 5 (0 percent power, 3 psig, 128 degrees F) and unit 2 in mode 5 (0 percent power, 70 psig, 120 degrees F),
the suspended ceiling in the Operations locker room, elevation 732. Control Building, was found damaged in three places. The suspended ceiling provides 1 1/2 hours fire barrier for the area, and the damaged ceiling area rendered the fire barrier nonfunctional.
Upon discovery of the nonfunctional fire barrier, a fire breach was issued, and an hourly fire watch was established.
However, the damaged ceiling was not repaired within the seven days, and the seven-day limit allowed by Technical Specification (TS) Limiting Condition for Operation (LCO) 3.7.12 was exceeded requiring a special report.
The above described suspended ceiling was damaged in three places by employees working above the ceiling. The three damaged areas of the ceiling are small ano irregular in shapes. The biggest damaged area of the ceiling is approximately 40 square inches.
CAUSE OF EVENT An employee, or employees, working above the suspended ceiling have accidentally stepped on the ceiling at three different places. The repair to the damaged ceiling was not completed because material for the repair could not be obtained within the seven-day limit allowed by the LCO.
ANALYSIS OF EVENT This event is being reported in accordance with the requirements of action statement (a) of LCO 3.7.12.
A roving fire watch was established to inspect the Control Building, elevation 732, Operations locker room area on an hourly basis as required by the action statement of LCO 3.7.12.
The existing fire detection and fire suppression systems for the locker room area are operable and would actuate in the event of a fire. Therefore, there is no danger to safety-related equipment. No one was reported injured due to this event.
CORRECTIVE ACTION A roving fire watch at a regular interval of one hour was established and will be maintained until the damaged suspended ceiling is repaired and the fire barrier declared functional. A sign will be installed at the ceiling access points alerting individuals not to step on the suspended ceiling. This work is scheduled to be completed by January 15, 19S8. No further actions are planned.
