NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-259/87-33
50-260/87-33 AND 50-296/87-33)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted
by G. L. Paulk on September 1-30, 1987. The inspection included a review of
activities authorized for your Browns Ferry facility.
At the conclusion of
the insnection, the findings were discussed with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection included operational safety, maintenance
observation, surveillance testing observation, previous enforcement matters,
reportable occurrences, restart test program, review of licensee's management
goals and objectives specified in TVA's Nuclear Performance Plan, simulated
loss of offsite power testing and compliance with general design criterion,
design changes and modifications, diesel generators, administrative control of
special tests, information notice review, and followup of licensee's action on
a crack located in the main stack.
Within these areas, the inspection
consisted of selective examinations of procedures and representative records,
interviews with personnel, and observation of activities in progress.
The inspection findings indicate certain activities appeared to violate NRC
requirements in the areas of design control, maintenance activity control, and
QA document control.
The violations, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violations.
Violation B in the enclosed Notice of Violations pertains to
the failure of complying with plant drawing requirement for water proofing and
sealing of electrical conduits.
This appears to be a programmatic deficiency
not addressed by plant instructions.
The corrective actions to this violation
should address the programmatic concerns of sealing electrical equipment,
training of maintenance personnel, and correction of modification procedures.

Your attention is invited to unresolved items identified in the inspection
report.
The first unresolved item in paragraph 9.a. pertains to the cuality
assurance control of process computer software which is under evaluation by
your staff.
The other unresolved item in paragraph 11 concerns the adequacy
of the emergency diesel generator Surveillance Instruction to meet the intent
of General Design Criterion 18.
This item is being addressed in the
Surveillance Instruction Upgrade and testing programs.

We invite your attention to the concern noticed in the Microbiologically
Induced Corrosion (MIC) program.
Our review of this program indicates that no
manager has been in charge of program direction since the initial NRC review.
More management control is necessary to address known problems and initiate
program guidance and corrective actions.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosures are not subject to the
clearance procedures of the Office of Management and Budget issued under the
Paperwork Reduction Act of 1980, PL 96-511.
