SUBJECT: NRC INSPECTION REPORT 50-361/98-301; 50-362/98-301
Dear Mr. Ray:
From June 14-18,1998, an operator licensing certification inspection was conducted at your San
Onofre Nuclear Generating Station, Units 2 and 3, reactor facilities. The enclosed report
presents the scope and results of that inspection.
The inspection included an evaluation of eight applicants for senior operator licenses. We
determined that seven applicants satisfied the requirements of 10 CFR Part 55, and the
appropriate licenses have been issued. One applicant failed the simulator portion of the
operating test. The applicant's appeal and reapplication rights have been transmitted to the
applicant via separate correspondence.
During this inspection, we discussed with your staff two performance issues of concern
observed during the examinations. First, two of the four crews of applicants were significantly
challenged by a small-break-loss-of-coolant accident scenario. Second, several applicants had
difficulty meeting your expectations when responding to a diesel generator vital electrical bus
failure to energize. You should ensure that any training or knowledge deficiencies of watch
standers are properly addressed.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
