NRC SPECIAL INSPECTION REPORT 50-271/98-09 AND NOTICE OF

VIOLATION

Dear Mr. Maret:

From June 9-19,1998, the NRC conducted a special inspection at the Vermont Yankee

facility to evaluate the personnel actions and equipment response associated with an event

of June 9,1998,in which there was a reactor vessel high water level turbine trip and

reactor scram followed by an electrical transient about an hour later.

Overall crew performance was acceptable with some shortcomings noted surrounding the

circumstances of the electrical transient event. The team found your review of these

events effective and timely with some exceptions noted. Your review was sufficient to

identify causes of the reactor trip and the electrical transient and provide adequate

corrective actions to support a safe restart of the unit. However, deficiencies exist

regarding the thoroughness of your review on electrical bus 1 overcurrent relay calibration

setpoints.

Based on the results of this inspection, the NRC has determined that tvio widns of NRC

requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice)

and the circumstances surrounding it are described in the report. The first deals with two

examples of inadequate procedures for operation of the reactor feedwater pumps and for

re-energizing electrical bus 6. This violation is of concern because there was a challenge

to safety systems, i.e., in one instance, the 4160 v safety bus 3 was deenergized and, in

the second instance, primsry containment isolation occurred. The second violation deals

with inadequate corrective action for deficiencies found during protective relay calibration.
