Gentlemen:
This letter addresses the inspection of your facility at St. Paul, Minnesota, conducted by Mr. R. L. Cilimberg and Mr. D. Lynn of this office on September 28 to October 2,1987, and the discussions of their findings with Mr. John Manship and other members of your staff at the conclusion of the inspection.
The purpose of this inspection was to follow-up an allegation pertaining to qualification of welders and QA program implementation related to fabrication of spent fuel storage racks for the Vermont Yankee Nuclear Power Station. Areas examined during the inspection and our findings are discussed in the enclosed report. This inspection consisted of an examination of procedures and repre-sentative records, interviews with personnel, and observations by the inspectors.
During the inspection, it was found that implementation of your QA program failed to meet certain NRC requirements. The most serious of these appears to be missing material test reports from documentation packages. Two other nonconformances were identified in the areas of instrument calibration and QA documentation control, while a violation was identified for failure to specify 10 CFR Part 21 on procurement documents.
The welder qualification allegation could not be substantiated. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974.
You are required to submit to this office within 30 days frc-date of this letter a written statement containing:
(1) a description of steps that have bec. g will be taken to correct these items; (2) a description of 4teps that have be o or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed.
We will consider extending the response time if you can show good cause for us to do so.
