DUKE POWER COMPANY McGUIRE NUCLEAR STATION s. RESPONSE TO VIOLATION Violation 50-369/87-26-01 Technical Specification 6.2.2.f requires. that adequate shif t coverage shall be maintained without routine heavy use of overtime and states that under certain circumstances when substantial amounts of overtime are required,'an individual should not be permitted to work more than 16 hours in any 24-hour period nor more.. than 72 hours in any 7-day. period, excluding shift turnover time. Technical.' Specification. 6.2.2.f requires' that 'any deviation from the 'above guideline shall be authorized by the, Station Manager or his deputy, or. higher levels of management, in accordance with established procedures and ' with documentation of the basis for granting the deviation. Station Directive 2.0.10,' Overtime Authorization for Personnel Performing Safety Related Func-tions, Revision 2, which implements Technical Specification 6.2.2.f, requires that excessive overtime be authorized in advance and documented on a Request for Work Hour Extension form. Contrary to the. above, on five occasions during the month of June 1987, individuals.vorked overtime in excess'of the Technical Specification limit of 72 hours in a 7-day period without the required management approval, and on numerous occasions during the months of September and October 1986,, March 1987, and May through July 1987, Request for Work Hour Extension forms were approved after the excessive overtime had been worked. Station Directive 2.0.10 allows the Shift Engineer on duty to approve work hour extensions, although -Technical ' Specification 6.2.2.f requires this function to be per-formed by the Station Manager, his deputy, or higher levels of management. This is a Severity Level IV violation (Supplement I). REVISED RESPONSE: 1. Admission or denial of violation: Duke admits to the violation as stated. 2. Reason for the violation if admitted: f h The violation occurred because of inadequate programmatic control. The station directive that was used to implement the program was not followed . correctly. 3. Corrective steps which have been taken and the results achieved: Overtime requirements have been transmitted to supervision and staff a. personnel in staff meetings held on September 23, 1987. b. Station Directive 2.0.10 has been revised to require prior approval { of overtime for affected hourly Nuclear Production Departmant (NPD) workers (employees and vendors / contractors) in excess of Technical Specification limits. Approval is from the Station Manager or group Superintendent (direct reporting management staff) in the Nuclear l Production Department. 1
