On May 22,1998, the NRC completed an inspection at Quad Cities Nuclaar Power Station.

The enclosed report presents the results of that inspection. The purpose of the inspection was

to review actions taken by your staff, as specified in your letter dated April 29,1998, that

i

responded to our confirmatory action letter, dated January 16,1998. The conclusions of the

inspection were discussed with you, and those members of your staff identified in the enclosed

i

report, on May 8 and on May 22, at the Quad Cities Station and on May 13, during a

'

_ management meeting held in the Region lli office.

The inspection concluded that the Quad Cities safe shutdown analysis and supporting

l

procedures, when combined with the compensatory measures agreed to during the above -

'

meetings and documented in your May 22,1998 letter regarding the inspection, were

acceptable as a minimum interim methodology. This was based upon a defense in-depth

strategy where the compensatory measures aided la: (1) preventing fires from starting, .

(2) detecting fires rapidly so that they could be controlled and extingu;shed promptly, and

(3) assisting the operations staff to ensure that the safe shutdown procedures could be '

implemented. However, the NRC understands that these compensatory actions were

implemented on a short-term basis and that the Quad Cities Station intends to take actions to

bring the plant into full compliance with 10 CFR Part 50, Appendix R.

During this inspection, NRC identified several concems which require further information in

order to allow us to evaluate them. These include: (1) the effects of loss of 125 Vdc power;

(2) the effect of fire damage to non-safe-shut down equipment on the ability to achieve and

maintain safe shutdown conditions; (3) the acceptability of crediting automatic closure of the

main steam isolation valves; (4) the acceptability of assuming one spurious operation in any fire

area, regardless of the number, type or location of cables and circuits in the area; and (5) the

' acceptability of not having full detection and suppression over fire hazards in fire area TB-ll.

These issues are currently categorized as an unresolved item and are discussed more fully in

the enclosed report. We request that, within 60' days of this letter, you provide a justification on

.

. how each of the above issues complies with the requirements of Appendix R. For the fourth

item, we request that your explanation specifically discuss the automatic depressurization

valves and the effect of multiple valves opening on the available time line.
