10CFR 50, Appendix B, Criterion XVI, requires that measures be established to assure that conditions adverse to quality are promptly identified and correct-ed.
These measures shall assure that the cause of the condition is determined and corrective action is taken to preclude recurrence.
Contrary to the above, the licensee failed to take adequate corrective actions to preclude the recurrence of a violation of Technical Specification 6.8.1 (369,370/87-05-02) while performing Slave Relay Testing per PT/2/A/4200/28.
The A train control room chiller experienced a loss of heat sink protective l
trip on July 21, 1987, despite the corrective actions for a similar trip on February 4, 1987.
This is a Severity Level IV Violation (Supplement I).
RESPONSE
1.
Admission or denial of violation:
Duke admits the violation occurred as stated.
2.
Reason for the violation if admitted:
Procedure PT/1-2/A/4200/28, Slave Relay Test, was originally written to provide testing flexibility. After the first trip, it became apparent l
that the incident occurred as a result of too much flexibility in the procedure. A procedure review was initiated and many possible alignments were reviewed. As a result, procedure changes were made to reduce procedural flexibility and correct the problems that were identified.
Those sections of the procedure which were revised as a result of the first violation have not been associated with any other VC/YC trips to date.
I The procedure sections responsible for the most recent trip were not the same sect 1ons that were changed in the earlier incident.
The alignment l
used on 07/21/87 was considered unusual and valve RN-148A had been i
overlooked in the review.
The combination of built in procedural flex-ibility and the numerous alignments possible on the shared unit VC/YC system result in an increased potential for system trips.
3.
Corrective steps which have been taken and the results achieved:
l The Slave Relay Test Procedures, PT/1-2/A/4200/28, were changed to require the opposite train of the VC/YC system to be in operation during the test and that the train undergoing testing be shutdown.
