OPERATIONAL SAFETY TEAM INSPECTION REPORT N0. 50-341/87030

This letter forwards the report of a special inspection performed by an NRC

operational safety team over the period of July 27 to August 7, 1987, involving

the operational performance of the Fermi-2 plant, authorized by Facility

Operating License No. NPF-43, issued July 15, 1985.

The inspection tean was

comprised of NRC Headquarters staff, Regional Inspectors, a NRC Training Center

Instructor and an NRC consultant, not directly associated with the Fermi-2

project, having the objective of obtaining an independent view of the opera-

tional problems experienced at Ferini-2 since receiving an operating license in

July 1985.

At the conclusion of the inspection, the team's findings were

discussed at an exit meeting with you and those members of your staff identi-

fled in the enclosed inspection report.

The NRC team effort focused on:

the effectiveness of management oversight of

plant operations and in communicating the goals and objectives of programs

designed to correct operational problems to plant operating staff; the control

and effectiveness of plant operating procedures and practices; a review of

surveillance test programs and related procedures in conformance with Plant

-

Technical Specification requirements; the effectiveness of administrative

procedures and controls; organizational interfaces and coordination in support

of plant operation; the adequacy of safety reviews and the process for

'

proposing and implementing plant modifications and corrective actions; training

program effectiveness; and a review of programs for assuring quality in these

areas.

Inspection of these areas included audits of ongoing equipment opera-

tions and surveillance terts, and observation of control room shift crew

operations.

Several reportable events involving equipment failures that were caused by

personnel and procedures errors occurred during the inspection period.

The team inspection did not identify any single area which could be construed

to be the primary basis for the operational problems which have been experienced

at Fermi-2 since receiving an operating license in July 1985, and which

continued to occur during the inspection period.

Rather, the inspection

findings affirmed prior staff assessments that the problems at your station

encompass a broad range of plant activities, including operating practices,

administrative controls, surveillance, training, and the corrective action

process.

The Regional Administrator's letter to you dated August 14, 1987, summarized

the team's principal inspection findings.

Your actions in response to these

findings and other actions needed for restart were discussed during an October

5 and 6, 1987 meeting.

In the Regional Administrator's letter to you dated

October 9, 1987 authorizing plant restart, you were directed to prepare a

program to demonstrate the ability of the operating shifts to perform certain

routine operating and training evolutions in order to gain additional plant

experience.

You were also directed to submit detailed plans for improvement in

the plant's Technical Specifications and their interpretation and implementa-

tion, and to prepare a comprehensive report of your various improvement pro-

grams and commitments to the NRC.

Because of these upcoming submissions, we do

not ask that you separately respond to this inspection report.

However, we

request that you consider the team's observations as you develop these submis-

sions and as you continue implementation of your various improvement programs.

Some of the items identified by the team may be potential enforcement findings.

Any enforcement actions will be identified by Region III.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure

will be placed in the NRC Public Document Room in Washington, D.C., and in the

Public Document Room located at the Monroe County Library System, 3700 South

Custer Road, Monroe, Michigan.
