It also refers to the discussion of our findings with

Messrs. S. Quennoz and W. Cottle and other members of your staff at the

conclusion of the inspection.

The inspection focused on the effectiveness.of your quality verification

)

organizations to identify, solve, and prevent safety-significant technical

i

deficiencies in plant systems and operations. 'The inspection also measured

the effectiveness of line management in ensuring that identified quality

J

deficiencies are responded to promptly and completely. -Quality: verification

organizations and line management have responsibility for ensuring that -

quality is achieved and that significant technical problems are identified

and resolved. The existence of significant problems reflect on their

performance and commitments.

Our review indicates that the AP&L Nuclear Quality organization'has'been

effective in their performance of quality verification' activities in the

operations and plant modifications areas.

The staff involved in-these areas

are experienced individuals who are capable of conducting in-depth technical

verification activities. The audits, surveillance, and observations-

,

'

conducted in these areas are performance-oriented and have result-d in the

identification of significant issues which impact plant safety. k'e noted,

however, that the frequency of observations made in the ANO observation

program has decreased since the program started. We encourage you to make'

more observations, especially during back shifts and in.the control room.

Our review also indicates that the Plant Safety Committee reviews all

significant reports of abnonnal: t.onditions and. requires, as warranted,

further evaluations to determine root cause,~ corrective actions to preclude

recurrence, and deportability to the NRC. The. inspectors determined in-

several instances, notably in the maintenance area, that these evaluations.

are not as thorough as expected and often require lengthy periods.of time to

be performed. We are concerned that these lengthy. evaluations could lead to

l

deficiencies continuing to exist for longer than necessary and delays in -

l

meeting the reporting requirements of 10 CFR 50.73.

'

We encourage you to seek improvement in those areas of concern identified in

the enclosed report and to promote the involvement of all plant organizations

and personnel in the assurance of quality.

In accordance with 10 CFR 2.790(a), a copy of.this letter and the enclosures

will be placed in the NRC Public Document Room.
