December 14, 1976' PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-76-216 l

This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

POSSIBLE PERSONNEL OVEREXPOSURE l

The licensee reported that a physician working in the Rcdiation Therapy I

Department received a dose of 17.2 rem gamma exposure during the month of November 1976, as indicated by TLD badge. A Region V inspector was at the facility on December 13, 1976 to look into this matter. Neither the Region V investigation nor the licensee's investigation has established i

how or when the indicated exposure might have occurred. The physician had worked with the Co-60 teletherapy unit.and X-ray machines during the month but had received radiation exposures from no other sources 4

during the badge period.

Other physicians and technicians working in the program, and sometimes with the exposed individual, received 10 to 20 mrem during the same period.

There have been no observed equipment malfunctions during the i

period, and all equipment appears to be operating properly at this time.

The physician received no medical or dental X-rays during the period, and he works only at this hospital.

The investigation is continuing.

There has been no publicity and neither the licensee nor the Region plan to make a news release.

Initial notification was received by telephone call from the licensee to Region V at 5:30 p.m. on December 10, 1976. This information is current as of 4:00 p.m. on December 13, 1976.
