MONTICELLO INSPECTION REPORT 50-263/98007(DRP)
Dear Mr. Wadley:
On May 27,1998, the NRC completed an inspection at the Monticello reactor facility. The
enclosed report presents the results of that inspection.

During the 6-week period covered by this inspection, the conduct of activities at the Monticello
facility was generally characterized by safety-conscious operations, sound engineering practices,
and careful radiological work controls. The reactor startup from the refueling outage was
conducted in a controlled and deliberate manner. The decision to delay the startup because of a
cracked contro; rod drive withdraw line reflected conservative, safety-conscious operation.
No violations were identified during this inspection. However, we are concerned that operations
personnel did not document in a condition report or in the control room log that a control room
manometer for the reactor building differential pressure was not operating properly. A work order
also was not initiated. Although venting the instrument line corrected the problem in the short
term, the root cause, a leak in the test connection, was not determined until the problem recurred
a few days later. Operators accepted the discrepant condition and did not aggressively pursue
,
resolution.
Also, the recombiner system was returned to service without completing a post-maintenance test
on newly installed switches. The system tripped about a week later during a train swapping
evolution because of miswiring of the switches. Although the switches were not safety-related,
this event is of concern since established controls for performing post-maintenance testing and
returning equipment to service were not effective.
