Statement of Violation Criterion V of Appendix B, Title 10 CFR Part .0 and Technical 5 q Specification 6.8.1 require. .in part, that activities affecting quality i shall be prescribed by and. accomplished in accordance with procedures appropriate'to the circumstances. The licenses has-implemented these requirements, in part, in the- ASME Section XI Repair and Replacement program delinected in General Plant Procedure OPGP03-AE-0025 and "ASME Section XI Repair and Repa1 cement [ sic]' Program," Revision 1, April 27,.1987

Contrary to the above, on March 4, 1988, the licensee failed to adequately record material identification markings in that the heat numbers for the studs on Repair and Replacement Traveller No. 1-88-019, Valve IRH065C, dated March 3, 1988, were not recorded, as required. II. Houston Lighting & Power Position HL&P does not contest the violation since the material mark number was recorded in place of the heat number. However, it should be.noted that although the heat numbers were not_ recorded properly, the overall consequences of this action did not have any affect on quality or the .] validity of the associated work document. .The following explanation is ' provided. The ASME Section XI Repair and Replacement (R&R) Traveller is not a stand alone document, but is an integral part of a Maintenance Work Request (HWR). The MWR, in conjunction with the R&R Traveller, provides the information that allows material.to be traced to certification records which assure compliance with original construction requirements. MWR No. RH57765 provides this traceability in'two_different. ways. First, on page 2, it is required that the HL&P Part Number and Inventory Sequence Number be recorded for all material used. Second, on page-3, the QC inspector annotated Step 1.02 to require the verification of both the heat and mark numbers of each stud and. indicated what the numbers should be (although the numbers were reversed, quality was not compromised because verification of both numbers was required.) HL&P also has comments on the following portions of paragraph 6 on page 26 of Inspection Report 88-70.as they pertain to this violation. ~ '"...Further'more, the appropriateness of instruction Steps (IB) and (1D) is.cuspect, in that ASME Section XI' requirements for' recording:. material identification markings is not explicitly stated by the- instructions..~The instruction steps require recording the heat numbers of the studs and nuts. The intent the instructions should- require theimechanic to record the material markings-in their- entirety.- By doing so, the R&R' traveller provides; direct objective evidence as to the parts' compliance with the original' construction requirements and. assures usage of the correct material;and: traceability to the. certification records". i

' As previously stated, the information contained within the HWR, not just the R&R' traveller, provides the required traceability to .the ' certification records. While we agree that the proposed method would provide a " cleaner path", there-is no requirement to perform the activity in this manner. , " l ...An additional example-of apparent failure to follow procedures. related to the same Maintenance Work Request.(HWR) package ' (RH-57765) concerns the instructions for QC verification. The instructions provided in the HWR prerequisite section, Step 1.02, in part, followed by the annotation " Studs --Heat #550B [ sic), Mark ZB2, 16 pieces total; Nuts Mark-#V6B, F54. Step 3.03 stated " perform work in accordance with Section XI Repair and Replacement ' Traveler R&R No. 1-88-019." General Plant Procedure- OPGP03-ZE-0025, Addendum 1, "ASME Section XI Repair and. Replacement Program," Revision 1, dated April 27, 1987, provided instructions for the proper annotation of the R&R traveler on'an item-by-item basis. The instruction for Item No. 41, " Operations QA " stated- l 3 , ' " Check appropriate inspection / hold point boxes, and initial and

date when performed." The material identification markings and ! associated QC verification as formatted;and annotated'on Page 11 of the R&R' traveler (R&R No. . 1-88-019) implies that QC only verified material markings'on installed studs and nuts (1-5) and not on all studs and nuts (1-16) as required by the_HWR' package". . l l l- NL.89.026.05 , .x,- ,_._ _ - - _ _ - -, . -- - - - - - - - -- - %

and consistency. The segmenting of the form is not intended to indicate that a single. activity step had to be confined within the borders of a ningle segment. It was not intended that.when a single activity exceeded one segment'that verification initials be required to be entered in more than'one segment. The activity of verifying the heat numbers went beyond the boundaries of one segment. The action, however, was one of a single activity and therefore was only required to be initialed once. l Please note that in the above excerpt that Heat #550B should be Heat #660B. III. Reason for Violation ! Mechanical Maintenance personnel were not completely familiar with

ASME material marking conventions. IV. Corrective Action Taken and Results Achieved ! t Training relating to this event was provided to Maintenance Shop Craft personnel which emphasized the proper identification and l distinguishing of heat numbers and material markings. l q V. Actions to Prevent Recurrence Additional training relating to ASME Section XI requirements and l material identification is being developed for Mechanical Maintenance l personnel.
