Introduction:==

Zion Unit 'One was shutdown-on September 5,:1986 for a normal ~ refueling outage. The outage was completed on February 26,(1987; Steam Generator

^ activities planned'for this outage were Eddy Current Inspection, Steam Generator-Tube Plugging and Steam Generator Tube Sleeving.

The initial Eddy Current inspection plan was for 100% inspection aof. A,f B, L

C and 0 Steam Generators-Tube Plugging was to be performed: based on L

- inspection results and Sleeving would be performed based on the plugging 1

criteria and location of the' indication, E

f These activities were performed in accordance with approved contractor procedures.

' ~

No unexpected conditions were encountered with the'results of thel eddy current test. A slight increase in the number of. Tube Support Plate' indications was noted and is under investigation.with consideration being given to. increasing the scope of the next inspection to include'more of'these-locations for. Rotating Pancake Probe inspections.

l During this inspection a small number of tubes that previously showed small indications were found to have reduced or no indications this outage.

Tube Plugging:

Tube Plugging was performed on all four Unit One Steam Generators using 1

approved Combustion Engineering procedures.

Tubes meeting Technical Specification or Preventive Criteria were plugged using the Combustion Engineering Rolled Plug. Sleeving was used as an alternate to plugging and is I

discussed later.

The Combustion Engineering GENESIS remotely operated fixture was used for -

the plugging. process. All normal operations were performed using this process.

A total of 211 tubes were found to.be defective or met the preventive plugging criteria and were plugged or sleeved.

Plugging Criteria:

o Through-wall indications of 40% or greater (Tech. Spec.)

o Restricted Tubes (Tech. Spcc.)

o distorted Roll Transitions with Possible Indications (Preventive) o all " Squirrels" (unknown depth indications) (Preventive)

Tube Sleeving:

Steam Generator Tube Sleeving was used as an alternative to tube plugging I

and was performed using the GENESIS manipulator using approve Combustion Engineering procedures. Sleeving criteria is the same as the pluggir.g criteria with the additional condition that'the indication must be in a location that is. repairable by. sleeving.

Due to the length of the sleeves and manipulator size, all areas of the l

Steam Generator tubesheets were not accessible for sleeve installation.

In these instances, the tubes were plugged.
