We reviewed Mr. Berry's' response of'Octoberil, 1987 to the Notice.of Violation

issued with our Inspection Report No. 50-155/87019. Aseindicated in:the-

inspection report, we'had carefully' considered the circumstances.ofLthe event-

in light' of the guidance of' 10 CFR Part 2, Appendix C,- cited by,Mr. ' Berry.

We concluded then, as;now, that the issuanc'e of .theLNOV.was" warranted.

decision was taken because in this event three independent personnel errors

opinion that the cumulative personnel errors represented weaknesses in'the

operations program due to deficiencies in the' procedures and an apparent lack

of attentiveness of the operators to the job.

Mr. Berry stated that all corrective actions taken to prevent. recurrence have

been completed. These actions will be reviewed by the NRC Resident Inspector.
