1 APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company (LP&L)

i During an NRC inspection conducted January 9-13, 1989, violations of NRC requirements were identified. The violations involved failure to use the I
material specified on the design drawing, failure to impose 10 CFR Part 21 on a i
vendor providing basic components, and failure to implement procedural j
commitments.
In accordance with the " General Statement of Policy and Procedure i
for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violations i
are listed below:

Criterion V of Appendix B to 10 CFR Part 50 and the licensee's approved quality assurance program description require that activities affecting quality shall be prescribed by and accomplished in accordance with i
documented instructions, procedures, or drawings, of a type appropriate to
]
the circumstances.
Design Drawing 1564-1539, R26 for pressurizer surge line Sample Isolation Valve 2SL-V2505 specifies that the material for Item 11 (valve gland i
studs) shall be AStiE SA-193 Grade B6 or B7.
Contrary to the above, a valve gland stud for Valve 2SL-V2505 had been incorrectly replaced with ASMF SA-193 Grade B8 material.

Section 21.31 of 10 CFR Part 21 requires the licensee to specify on each l
procurement document for a basic component that the provisions of I
10 CFR Part 21 apply, j
Contrary to the above, since 1985, the licensee has not specified on procurement documents for basic components placed with the Okonite j
Company, that the provisions of 10 CFR Part 21 were applicable.

Criterion V of Appendix B to 10 CFR Part 50 and the licensee's approved l
quality ossurance program description require that activities affecting quality shall be prescribed by and accomplished in accordance with documented instructions, procedures, or drawings, of a type appropriate to the circumstances.
Contrary to the above, the licensee failed to implement procedures as shown in the following examples:
Contrary to Section 5.7.7 in Procedure N0EI-152, Revision 1, the licensee failed to identify shelf life requirements in Purchase Order WP016013 for Buna-N parts.

.i Contrary to Section 5.4.3.2 in Procedure VNT-7-021, Revision 1, the licensee failed to update at least five vendor technical manuals to incorporate data to support replacement AGASTAT relays installed j
during Modification SM1701.
Contrary to Section 5.2.1.4'in Procedure UNT-8-001,'the licensee failed to properly dedicate commercial grade replacement electrical resistors which had been procured on Purchase Order W117408.
This~is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted,
- 2) the corrective steps which have been taken and the results achieved,
- 3) the corrective steps which will be taken to avoid further violations, and
- 4) the date when full compliance will be achieved. Where good'cause is shown, 1
consideration will be given to extending the response time.
