PEACH BOTTOM ATOMIC POWER STATION STATUS REPORT FOR THE PERIOD September 24 - October 8, 1987 Enclosed is the Peach Bottom periodic status report from the NRC Resident Office at Peach Bottom. Three NRC resident inspectors monitored activities at the plant during the report period.

These status reports are intended to provide NRC management and the public with an overview of plant activities and NRC inspection activities. Subsequent  !

inspection reports will address many of these topics in more detail.

PEACH BOTTOM ATOMIC POWER STATION STATUS REPORT FOR THE PERIOD SEPTEMBER 24 - October 8, 1987 1.0 Plant Status As of 8:00.a.m...on October 8, 1987, Unit 2 and 3 are in cold shutdown per the March 31, 1987, NRC Order with coolant temperatures at approximately.

150 degrees Fahrenheit.

2.0 Facility Operations Summary Unit 2 is completing a refueling outage which began March 13, 1987. Final work.is being performed to support the reactor pressure vessel hydrostatic test. Unit 3 has been in a cold shutdown condition.per the March 31, 1987, NRC Order since April 1, 1987. The Unit 3 recirculation system pipe replacement outage began on October 1. Unit 3 reactor vessel disassembly is in progress.

3.0 Items of Special Interest 3.1 Public meetings were held on September 24, 1987, in Harford County, Maryland, and York County, Pennsylvania to receive public comments on the Philadelphia Electric Company (PEco) restart plan.

3.2 On September 25, 1987, the Institute for Nuclear Power Operations (INPO) completed a two week visit to perform its periodic operational assessment. The team will complete its evaluation on September 25, 1987.

3.3 Philadelphia Electric representatives presented their health physics program for the Unit 3 replacement outage to the NRC Region I staff on October 7, 1987. Methods to ensure that personnel radiation exposures are kept As Low As Reasonably Achievable (ALARA) were discussed.

4.0 Other Items of Interest.

4.1 On September 29, logic power to HPCI was lost when a fuse to a battery charger blew. The event occurred when the E 1 diesel generator energized the E12 4KV emergency bus. The licensee reported this event to the NRC based on the fact that HPCI is a single train

.. system and its failure whether the Unit is operating or shut down is reportable. ',

W i.y 4.2 A group-II: containment. isolation and 'oss'of-shutdown cooling occurred on' Unit 3:on October.5. The. apparent cause of the isolation was a momentary loss of power to the panel which supplies-logic power. 3A test engineer was troubleshooting a problem with a high pressure service water pump breaker and inadvertently opened s the compartment for the'4KV bus' potential transformer. This caused

.an undervoltage signal whichiopened,the load center breaker causing loss offpower to the containment isolation logic power. The test:

engineer reclosed the compartment door and the load center breaker reclosed.in~3 seconds as designed. The licensee reset the isolation and restored shutdown'. cooling.

4.3 'On September 30,'a contractor employee tried to enter the protected area while intoxicated. Licensee security guards administered sobriety tests which the individual failed. His supervisor was notified and his security badge was pulled to deny protected area access. The contractor eventually terminated the individual's employment. In an unrelated case, on October 4,~a contractor-employee found a small plastic bag containing a white powdery substance in the parking lot outside of the protected area. He

- turned the bag over to security who then turned it over to'PECo Claims Security for analysis. Five. hours later a security guard-found a small pile of white powder on the floor in. the men's rocm .

inside the guard house. This area is outside the protected area but I beyond the protected area search section. The pile was scooped up and turned over to PEco claims security for analysis. Licensee preliminary testing was performed on both substances the morning of October.5; each tested positive as cocaine. The licensee notified the Pennsylvania State Police and will transport the illegal drug to the state '1aboratory for more accurate analysis disposal.

4.4' On September 26, while applying a system blocking permit on the E-4 Diesel Generator (DG) cardox fire protection system, the licensee found the electro-manual pilot cabinet (EMPC) block valve stem to be separated. The EMPC block valve is a 3/4 inch Jamesbury manufactured valve. With the valve operator stem separated from the valve disc, the valve.was closed. This prevented automatic and manual actuation of the cardox system for the E-4 DG room. Both units 2 and 3 are in ,

cold shutdown and the E-4 DG is not required to be operable. The licensee initiated a fire watch for the E-4 DG room and repaired the valve.

The resident inspectors are following the licensee investigations and corrective acetions regarding these occurrences, i

, October;2,.-1987. Areas. inspected included operational . safety, shutdown order commitments, radiation. protection,. physical security, control room activities'and demeanor, reportable events, surveillance testing,' outage

-activities,. maintenance, and' outstanding items. No violation of NRC requiremer.ts were observed, however,,the number of shutdown cooling system.is_olations was an item'of concern. The licensee was requested to review these.1,solations and provide a written response describing the root causes and corrective actions taken to reduce the _ frequency of these occurrences.

Reactor operator' license exams were' administered by NRC examiners on

.: October 5'to October 7, 1987-Q A Regulatory' Effectiveness Review (RER).was performed from September 28 ,

to October 2,.1987 to evaluate the plant physical security program. The review results will be available at a later date.
