(a) Letter from A Bert Davis, Regional Administrator, Region III, USNRC, Notice of Violation and Proposed Imposition of Civil Penalty, NRC Inspection Report No. 50-282/87011 (DRP), September 18, 1987 (b) Letter from C E Larson, NSP, to Director Office of Enforcement, USNRC, October 1, 1987 On September 18, 1987, Reference (a) was issued by the NRC Region III Administrator. The Notice of Violation concerned inoperability of No. 11 Safety Injection Pump when, on June 18, 1987, its motor breaker was found not fully racked in.

Detailed information related to this event was provided in Licensee Event Report 50-282-87-009 dated July 17, 1987.

In accordance with instructions given in the Notice of Violation. the following response is offered.

fully racked in on June 18, 1987 and had probably been in that condition since May 27, 1987.

Reasons for the Violation j

Root cause of the event was failure to leave the breaker completely racked I

in.

This could have resulted from failure to properly rack the breaker in or inadvertent disengagement of the locking lever during tool removal.

Corrective Steps That Have or Will be Taken to Avoid Further Violations l

The following corrective actions have been taken:

- All other 4KV breakers were checked and found to be properly racked in.

l

- The breaker was mechanically and electrically inspected; no abnormal conditions were found.

- A Shift Supervisor was assigned to independently investigate the event.

This investigation focused on the involvement of operators in this event and included interviews with all the operators on shift at the time of the event.

Administrative controls for independent verification of breaker condition were tightened.

Specifically, any safety-related 4KV breaker will be operationally tested after being racked in.

The plant technical staff will be evaluating possible modifications to these breakers which which will provide positive assurance that the breakers fully racked in without the need for an operational test.

- Procedure changes were made to better control the status of safety injection pump motor breakers.

- Procedure changes were also made to improve the methods for independent verification of breaker status.

All operators have received hands-on training on these revised procedures.

Date When Full Compliance Will Be Achieved We believe that the corrective actions described above will prevent future occurrences.

In particular, operational testing of breakers after racking them in will provide positive assurance that they are properly racked in.

Full compliance has been achieved.
