This refers to the routine resident safety inspection conducted by Messrs
H. Eichenholz
and
M. Markley
of
this
office
on
November 15, 1988 -
January 17,1989, at the Yankee Nuclear Power Station, Rowe, Massachusetts.
The inspection consisted of observations of activities, interviews with per-
sonnel, independent measurements and document reviews.
The results of the
inspection are described in the NRC Region I Inspection Report enclosed with
this letter and were discussed with Mr. N. St. Laurent of your staff.
Based on the results of this inspection, it appears that two of your activities
were not conducted in full compliance with NRC requirements, as set forth in
the Notice of Violation, enclosed herewith as Appendix A.
The violations con-
cern the failure to meet 10 CFR Part 20.203 (c)(1) requirements to conspicu-
ously post a high radiation area, and the failure to meet 10 CFR Part 50,
Appendix B,
Criteria XVI requirements for establishing measures that would
ensure the proper documentation and reporting to appropriate levels of manage-
ment
of
significant
conditions
adverse
to
quality
involving
design
deficiencies.
These violations have been categorized by severity level in accordance with the
" General Statement of Policy and Procedure for Enforcement Actions", 10 CFR Part 2, Appendix C.
You are required to respond to this letter and, in so
doing, should follow the instructions in Appendix A.
However, we note that no
response to Violation A of Appendix A is required because immediate corrective
actions and actions to preclude recurrence were both timely and adequate.
