APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Docket: 50-313 Arkansas Nuclear One, Unit 1 License: DPR-51 During an NRC inspection conducted during the period June 1-30, 1987, a violation of the NRC requirements was identified. The violation involved failure to follow procedural requirements during a surveillance test.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

l Technical Specification 6.8.1 requires, in part, that written procedures i shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972.Section I.4 of this Appendix includes procedures for exercising equipment which is normally idle, but which must operate when required.

Procedure 1106.06, Supplement III, Emergency Feedwater Valve Stroke Testing, has been established in accordance with this Technical Specification.

A note following Step 2.39 of this procedure requires that, "A dedicated operator must remain in the vicinity of CV-2870 when it is open to manually close the valve in the event it does not close electrically."

Contrary to the above, on June 16, 1987, a dedicated operator was not stationed at CV-2870 as required when cycling this valve during the performance of Procedure 1106.06, Supplement III.

This is a Severity Level IV violation. (Supplement 1) (313/8718-01)

Pursuant to the provisions of 10 CFR 2.201, is hereby requirea to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and if applicable, a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice. This reply, should be clearly marked as a " Reply to a Notice of Violation" ano should include for each violation: (1) the reason for the violation if adJ.itted, (2) the corrective steps that ha'.'e been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
