This refers to an investigation performed by the NRC Office 'of Investigations (01).concerning the installation of Concrete Expansion' Anchors (CEAs) at your facility. A copy of the synopsis of Investigation Report 3-84-001'is~ enclosed with' this letter. The investigation found that three craftsmen willfully violated a Baldwin' Associates (BA) procedure concerning the' installation of
, CEAs and plates.
A Notice of Violation (N0V) was issued on January 27,'1984, which addressed the improper installation of.CEAs. The Severity level IV violation was adequately -
addressed by.you from a. technical standpoint in responses dated March 16 and April 16, 1984. However.. in view:of.the NRC:01. conclusion that the events -
occurred.as a result < of. willful violations on the part 'of craftsmen at your facility,.you may wish to review your. corrective actions to see if further action is. necessary.

~ SYNDPSIS On December 20, 1984, NRC Region Ill (Rill) requested an investigation into ' allegations that craft personnel employed by.Baldwin Associates (BA) at the Clinton Power Station (Clinton) had installed concrete expansion anchors (CEA) on safety related hang:rs in violation of BA procedures. 29, 1983, an Institute of Nuclear Power Operations (INPO) On November Evaluation Team identified a number of irregularities in the installation In early December '1983, the NRC Senior Resident of CEA and base plates. Improper techniques found on the installation included tack welding of a CEA nut to the plate and wire wrapping of the CEA. One BA craftsman alleged that his foreman had knowledge of the use of improper CEA installation' techniques. BA craft supervision denied knowledge of the improper techniques and the existence of production quotas. The investigation revealed that in 1981 - 1982, the time the improper techniques were used to install the CEA, craft had not received any type of training in their installation.
