Violation 50-369/87-26-01 i

Technical Specification 6.2.2.f requires that adequate shift coverage shall be maintained without routine heavy use of overtime and states that under certain circumstances when substantial amounts of overtime are required, an individual f

should not be permitted to work more than 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> in any 24-hour period nor more than 72. hours in any 7-day period, excluding shift turnover time.

Technical Specification 6.2.2.f requires that any deviation from the above guideline shall be authorized by the Station Manager or his deputy, or higher levels of management, in accordance with established procedures and with documentation of the basis for granting the deviation.

Station Directive 2.0.10, Overtime Authorization for Personnel Performing Safety Related Func-tions, Revision 2, which implements Technical Specification 6.2.2.f, requires that excessive overtime be authorized in advance and documented ou a Request j

for Work Hour Extension form.

Contrary to the above, on five occasions during the month of June 1987, individuals worked overtime in excess of the Technical Specification limit of 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> in a 7-day period without the required management approval, and on numerous occasions during the months of September and October 1986, March 1987, and May through July 1987, Request for Work Hour Extension forms were I

approved after the excessive overtime had been worked.

Station Directive 2.0.10 allows the Shift Engineer on duty to approve work hour extensions, although Technical Specification 6.2.2.f requires this function to be per-(

formed by the Station Manager, his deputy, or higher levels of management.
