Review of' Peach Bottom Commitment to Excellence Action Plan Your September 28, 1987 reply to our questions about your Peach Bottom' Commitment to Excellence Action Plan (CTEAP) has not addressed a fundamental concern regarding the past inability of Philadelphia Electric Comp'any (PECO) to self-identify problems, and implement timely and effective corrective actions. We understand from your response to question 1-6 that changes are being contemplated regarding operations management structure whi may.in part address this concern.
However, we feel that it is important for,ou to-fully understan;i our-concerns before you propose changes to your operations management structure or the CTEAP.
.0ur concerns have.been heightened by the quality of your September 28, 1987
- r.esponse..For example, your response to question 1-28 to explain the connections between the root causes of the problems at Peach Bottom and the category 1 tasks which are intended to correct the root causes was unclear and inconsistent.
Inconsistency was found between the attribution of tasks to root causes in the
" detailed" task descriptions submitted in response to question 2-1 in Attachment 2 and the lists provided in the response to question 1-28 in Attachment 3.
The
" detailed" task descriptions in Attachment 2 do not contain sufficient detail to understand the relationship between the tasks and the cited root cause. Your reasons for proposing some tasks be accomplished after restart'in response to questions 1-1 to 1-26 in Attachment 1 were in many cases questionable.
Further,.
there is very little-in your CTEAP which addresses concerns about the capabilities of-corporate management to identify problems at the nuclear facilities promptly and independently of plant line management reporting, to' analyze and address such. problems quickly,- and to assess and evaluate the results' of such actions.
ITheNRCstaffisdeferringfurtherreviewoftheCTEAPpendingreceiptofa plan revision which addresses this fundamental concern.
