NOTICE OF VIOLATION

(NRC AUGMENTED INSPECTION TEAM REPORT NOS. 50-338/87-24 AND

50-339/87-24)

This refers to the Nuclear Regulatory Commission (NRC) augmented inspection

conducted by Floyd S. Cantrell on July 15 - August 14, 1987.

The inspection

included a review of activities associated with the Unit 1 steam generator tube

rupture at the' North Anna facility. The report documenting this inspection was

sent to you with a letter dated August 28, 1987.

The violations described in the enclosed Notice of Violation involve a failure

.

to perform the actions of an alarm response procedure, a failure to take

corrective action prior to restoring a failed radiation monitor, a failure

to conduct.a safety committee review of abnormal conditions prior to restart,

and a failure to perform adequate channel and source checks of a radiation

monitor.

In accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Act' ions," 10 CFR Part 2, Appendix C (1987), the violations

described in the enclosed Notice have been classified as Severity Level IV

violations.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including jour proposed corrective actions and the results of future

inspections, the NPC will determine whether further NRC enforcement action is

I

necessary to ensure compliance with NRC regulatory requirements.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosure are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
