ENFORCEMENT CONFERELCf 4ANAGEMENT MEETING
Gentlemen:
This refers to the meeting held with you and members of your staff on
January 26, 1989, at the Region V office in Walnut Creek, California.
Subjects discussed during the meeting are described in the enclosed meeting
report.
We concluded that with regard to the Main ~ Feedwater pipe material procurement -
to replace eroded pipe in 1987, you failed to apply an adequate level of
Quality Assurance in the procurement of the materials.
Further, in the
meeting it was concluded that you will revise your procurement practices for.
commercial grade materials and equipment, and you will assure that the
materials used for the Main Feedwater pipe replacement are' appropriate for
their application by chemical and physical testing, where possible.
Since the enforcement conferenca, you have determined that the information
provided during the conference did not present a complete, accurate
representation of the material testing to that date. This appears to be
another example of the failure of the QA department and management to clearly
focus on this important issue.
Further, I find these continuing problems with
the procurement of material particularly vexing in light of a 1985 violation
relating to the same practices.

Based on your telephone conversation on February 13, 1989 with
Mr. R. Zimmerman of my staff, we understand that by February 21, 1989 you will
provide your_ written safety assessment of the acceptability of the feedwater
'
piping for continued operation.
Please include in this assessment the items
specified in Enclosure B to this letter.
,
PGE efforts in the radiation protection area, as presented in the management
- meeting, indicate a clearly improving perfonnance trend. Also, the efforts
,
and programs in the event assessment and quality assurance areas show some
potertiel for improvements.
Finally, it is my understanding that you are
fully' committed to significantly improving perfonnance in these areas.
