St. Mary's Hospital respectfully requests the renewal of their special nuclear materials license SNM-1804 to implant plutonium - 238 nuclear powered pacemakers.
Reference to NRC letter dated 29 September 1982, St. Mary's Hospital is requesting n..ewal of this license under Item II, paragraph A.
We need to continue the follow-up procedures for six (6) patier.is with implanted plutonium pacemakers, manufactured by the Coratomic Company.
St. Mary's currently has no plutonium pacemakers in its possession and does not anti-cipate the use of these pacemakers in the future. However, as long as the license has to remain in effect, we would like to maintain this as an option if the cardiolo-3g gists deem necessary.
All of the plutonium pacemakers used by this institution have been from the Coratomic O
Company.
To this date, all installed pacemakers have been accounted for, either by l
g being actively utilized or having been returned to the manufacturer.

Page 2 All records of patients with plutonium powered cardiac pacemakers will be maintained by the Department of Cardiology and submitted to the Medical Radioisotope and Radia-tion Safety Committee on an annual basis.
All patients with plutonium powered cardiac pacemakers are infonned of the proper telephone procedure for contacting St. Mary's in the event of change of address, or removing of the installed pacemaker.
The written instructions for the hospital tele-phone operators are enclosed with this application.
