1 c) We wish to be authorized for 20 pacemakers. This is the number for which we are presently authorized and includes three units presently. implanted in patients.

These pacemakers are Cordis Nuclear Omni-Stanicor Model 184A units, each containing no more than 225 mg Pu-238.

j d) The activity to be conducted under this license is the continuation of previously l

authorized activities.

This includes receiving and implantirg pacemakers, following patients with implanted devices, and returning explanted pacenckers to the manu-facturer for proper' disposal.

e) The list of physicians involved in our pacemaker program may be updated by removing tbn following indivictuals from that list:

Samuel L. Kalush, M.D.

James F. Hoffman, M.D.

David G. Kamper, M.D.

W. D. Johnson, M.D.

f) We wish to make no changes in the commitments contained in the correspondence listed in Condition 16 of the existing license.

St. Luke's Hospital requestt a five year renewal of NRC License number SNM-1638. This license presently allows the Hospital to implant FU-2?8 i

Nuclear-Powered Cordis Omni-Stanicor Model 184A Cardiac PacemaP.ers.

The physicians of this institution wish to continue to receive, store, implant, and dispose of the nuclear-powered pacemakers. To this end, the hospital requests that:

The present possession limit (20 individual sources not exceeding 225 milligra.ms PU-238 each) be continued.

2.

The physicians listed in the original license application be authorized to use this special nuclear material for "implan-tation in humans as a commnent of Cordis Omni-Stanicor l

Model 184A nuclear-pc J cardiac pacemakers for clinical l

evaluation purposes."

3.

The list of authorized physician users be amended by dropping Lowell Davis, M.D., and H. D. Friedberg, M.D.

These individuals are no longer practicing at St. Luke's Hospital.

The Hospital will cantinue to abide by all the conditions of the present license (SNM-1638) as well as the statements, representations, and procedures contained in the original license application. This information is contained in lotters to the NRC dated February 4,1976 (signed by G. Edwin Howe, President, and John C. Manley, M.D.), and May 10,1976 (signed by G.

Edwin Howe).
