to this letter provides a status on the commitments made to the Augmented Inspection Team on June 20, 1987, as documented in our letter PY-CEI/0IE-0276 L. is a summary of the drawing consistency review for the C71, C51, and B21H systems as committed to in our June 20, 1987 letter.

r PY-CEI/0IE-0277 L Commitment CEI will modify the power supplies to the MSIV solenoids. This modification will preclude an MSIV isolation from the loss of a single RPS power supply.
This modification will be completed prior to startup following the July 12, 1987 outage.
CEl will continue to discuss this proposed modification with the NRC staff.
Status A Design Change Package (DCP) has been initiated to modify the power supplies to the MSIV solenoids. As discussed in the commitment this change will preclude an MSIV isolation from the loss of a. single RPS power supply. CEI presently intends to install this modification prior to startup following the July 12, 1987 outage. CEI personnel have discussed this modification with both site NRC personnel, and members of the NRR staff. The modification package has been given to the~ Perry Nuclear Power Plant NRC Resident Office.
Commitment CEI will add a note to the appropriate plant instruction to require the Operator to shut the MSIVs using the remote manual switches, if the MSlVs close as a result of an RPS power loss.
This action will be complete prior to the plant startup.
Status CEI changed the appropriate plant instructions to require the operator to shut the MSIV's using the remote manual switches, if the MSIVs close as a result of an RPS power loss. The action was completed on June 20, 1987 prior to plant startup.
Commitment CEI will provide improved permanent barrier separation in the RPS relay cabinets (H) 3-P691, H13-P692, H13-P693, and H13-P694) prior to startup following the July 12, 1987 outage.
Status Engineering is presently evaluating the internal wiring in the RPS relay cabinets to determine necessary improvements.
The improvements will be installed prior to startup following the July 12, 1987 outage.

Commitment CEI will complete a consistency review of General Electric drawings versus GAI drawings for C71, C51, and B21H systems. While this review will be essentially completed by June 22, 1987, all attendant changes to the drawings will be completed by June 26, 1987. A summary of the results of this review will be sent to the NRC.
Status CEI completed the consistency review for the three systems on June 22, 1987 (See Attachment 2).
This review indicated that there were no additional drawing discrepancies which affect wiring, power supply, or component / system function.
The drawing changes to the MSIV pilot solenoid power supply drawings and to the minor discrepancies found during the consistency review have been approved. This item is considered completed.
Commitment CEI will satisfactorily complete surveillance testing of the normal RPS power supply output breakers prior to starting the plant.
CEI will change out the EPA cards of the normal RPS "B" power supply prior to startup following the July 12, 1987 outage.

The.RPS' power supply assignments for the inboard and' outboard Main Steam I
Isolation Valves (MSIV) are shown on gal drawings B 208-013 Sheets HOS, H10, and'
' H11. These drawings are b'ased on GE elementary wiring diagrams 828E445CA, Sheets 4,10, and 11. Revision 1 of GE drawing 828E44SCA, Sheet 4 shows Figure' 6
- (outboard MSIV's) being supplied by Panel 1C71-P001 and Figure 7 (inboard MSIV's)

Revision 2 reversed the power supplies between Figures 6 and 7. Figure 6 is now supplied by 1C71-P002 and Figure 7'is supplied by 1C71-P001. While other changes
'on the GE drawing were clearly identified with ba& circles, the change of panel assignment on Figures 6 and 7. were not. Consequently this change was not p
, incorporated on the equivalent gal drawing B 208 013, sheet H05. This resulted in l-the two solenoids controlling each MSIV being supplied from the same power source rather than from two different power sources as shown on GE drawing 828E445CA, Sheet 4, Revision 2.
r Therefore, when the Electrical Protection Assembly (EPA) for Panel 1C71-P001 trippe'd, power was lost to the two controlling MStV solenoids instead of only one solenoid thereby causing the MSIV to close resulting in the Reactor Protection L
System to initiate a scram.
Based on a detailed review of drawings B 208 013, Sheets HOS, H10 and H11, B 208-e 040, Sheets A09 and A10, and B 208 041, Sheets B01 and 802, the drawing anomaly
-for the RPS power supply to the MSIV solenoids was determined to be an isolated case. Ho' wever, in order to further confirm the randomness of this anomaly y
between the gal and GE design drawings, an in depth review on all systems supplied by RPS distribution panels 1C71 P001 and 1C71 P002 was performed.
These systems comprise C71 A - Reactor Protection System, C71B Reactor Protection System MG Sets, B21H Nuclear Steam Supply Shut-off System, and C51 A - Startup Range Neutron Monitoring System. The objective of the review was to determine if the design configuration depicted on the latest GE elementary wiring diagram (EWD) had been correctly and completely transferred to the corresponding gal

elementary wiring diagram.

confirmed that there were no additional drawing discrepancies which affect wiring, power supply, separation or component / system function. No additional physical wiring, termination or control changes are required except for the RPS power supply changes to 1C71-P001 and 1C71-P002.
- A total of 75 gal elementary diagrams were reviewed along with the corresponding GE EWD's. Of the 75 drawings,51 drawings had no discrepancies between the GE and GAI elementaries. On three drawings differences existed between the GE drawings and gal drawings B21-H07, C51-A26 and C71-A12. The gal drawings reflected the correct design. Other than the error on Sheet 821-H05 which was the initiating problem causing the scram, there were no other power supply, separation, or termination or wiring discrepancies identified.
l The remaining discrepancies involve minor differences in notes, instructions, and schematic format which do not affect wiring / function, separation, or design intent.
i Therefore, the conclusion is that the anomaly between the GE and gal elementary for the RP5 power supply to the MSIV solenoids was an isolated case.
