CONDITIONS

10. Licensed material shall be used only at 550 W. 64th Avenue, Anchorage, Alaska.

11. Licensed material shall be used by, or under the supervision of, Wayne Stolt,

Doug Minert, or Tom Sexton.

l

12. The licensee shall assure that each device distributed contains instructions

and labels as required by 10 CFR Sections,32.51 and 32.51a. Labels and

instructions provided by the manufacturers with sealed sources and devices

shall not be removed or altered by'the licensee. ~

13. The licensee shall maintain' transfer records and submit _ quarterly transfer

reports to NRC and Agreement States in accordance with 10(CFR Section 32.52.

14. The licensee shall, conduct a physical inventory every.six (6)" months to account

for all sealed sou'rces received and possessed under;the license. The records of

the inventories shall be maintained for two (2), yea'rs from the,date of the

inventory for inspection by the Commission,:and shall include the quantities and

kinds of licensed material
