This letter confirms the telephone conversation on September 8,1987 between-Mr. E. G. Greenman and others of this office and Mr L. Storz of your staff regarding the events surrounding the reactor trip and subsequent equipment malfunctions at the Davis Besse facility on September 6 and 7,1987.- With regard to the matters discussed, we understand that you will:

1. Determine the cause of the reactor trip.
2. Determine the cause of each equipment malfunction which occurred i

coincident with or subsequent to the September 6,1987 trip, including an l assessment of historical reliability.

3. Take those actions necessary to ensure that complete documentary evidence of the "as found" condition of equipment which malfunctioned is maintained.  !
4. Not disassemble the failed main steam safety valve without the prior concurrence of the Augmented Investigation Team (AIT) Leader, N. J. Chrissotimos, or W. G. Guldemond of my staff.

5. Except as dictated by plant safety, advise the NRC AIT Leader prior to conducting any troubleshooting activities. Such notification should be provided soon enough to allow time for the team leader to assign an inspector to observe activities.
6. Review operator and shift personnel actions following the event to determine if those actions were in accordance with your procedures and policies.
7. Evaluate the adequacy of the normal, abnormal, and emergency procedures used during the event and subsequent plant stabilization.
8. Evaluate the timeliness and completeness of the information reported to the NRC pursuant to 10 CFR 50.72 regarding this event.
9. Submit to NRC Region 111 a formal report of your findings and conclusions within 30 days of receipt of this letter.
