M. Terc during the period Novemoer 15-17, 1988 (Inspection 88-22). The inspection included the implementation of the emergency plan and procedures during the annual ) emergency response exercise (FOSAVEX-88). The following is Public Service Company of Colorado's (PSC) responses and schedule of corrective actions planned for each admitted weakness, as requested. 267/8822-01: (NOTE: The specific items within this weakness have been . numbered for discussion identification.) "In general, the control room staff performed efficiently; however, there were fourteen instances that notification messages were not completed properly, as follows: , At the Notification of Unusual Event (NOVE): 1) The Emergency Coordinator did not sign the form indicating his approval prior to the release of the NOUE information. 2) The plant management co1 tact nane was not entered on the NOUE notification form. 3) The time was not specified en the NC'A notification form. f h acn , wn , u .we /r-" r i

P-89023 -2- January 25, 1989 '4) The caller's name was not entered on the NOVE notification form. 5) The time of the verification callback was not entered on the NOUE notification form. 6) The verification scheme was invalidated.because the control room operator provided the caller with ~the confidential verification telephone number.. At the Alert Declaration: 7) The date was not specified on the alert declaration form. 8) The emergency action level (EAL) number was not included on the alert declaration form. l 9) The stack monitor activity level was not specified or. the alert declaration form. i 10) The notifier was not 'specified on tre alert declaration form. 11) The emergency classification was not identified on the alert declaration form. 12) The wind direction was erroneously listed " west" instead of l "from the west" on the alert declaration form. 13) The Emergency Coordinator did not sign the alert declaration i . l form indicating his approval prior to the release of the information. 14) The Emergency Coordinator failed to specify on the alert declaration form the affected areas in the " seek shelter" ' message issued as a protective action recommendation." 1

. . l l l A l ' P-89023 -3- January 25, 1989 - pSC Response: Items 1, 2, 3, 4, 5, 7, 10, 12 and 13 are admitted weaknesses. These weaknesses will be reviewed by all licensed operators via onshift training being conducted at this time. Also, a RERP-CR review team is being formed to review the existing notification forms for adequacy, reduction in complexity, and to incorporate comments from the licensed operators. The onshift training is scheduled for completion by February 24, 1989. The RERP-CR review team is scheduled to complete its review and provide recommendations for revision by February 28, 1989 (any valid comments received from the operators during the onshift training will be incorporated). Processing and approval of any revisions determined to be necessary, required training, and subsequent implementation of the procedure revisions is scheduled for completion by April 28, 1989. Item 6 is an admitted weakness. The Weld County Emergency Communications Center has been notified on several occasions of Fort St. Vrain's Control Room phone numbers for call-back verification numbers, however, the 911 operators are trained to automatically ask for a telephone number of the caller, therefore, the Fort St. Vrain licensed operators will be informed not to provide that number if asked and to refer the 911 operator to their Fort St. Vrain instructions. Additionally, a hardware enhancement to the Weld County communication process is underway. This enhancement includes the installation of a direct, dedicated, microwave " hotline" between the Fort St. Vrain Control Room and the Weld County Emergency Communications Center. Construction on the Weld County end of the new system is approximately eighty percent complete, and final - installation and testing is currently scheduled for completion by February 28, 1989. This system will eliminate the need for a call-back verification from Weld County. The information referred to in Items 8 and 9 is not required on the Fort St. Vrain notification forms, therefore, PSC aces not agree that these items represented weaknesses in accordance with existing procedures. However, the RERP-CR procedure review team will evaluate the need for such information and make appropriate recommendations by February 28, 1989. With respect to Item 11, review of the forms used during this inspection indicate that all were properly marked as to the level of classification. The particular form referred to was, in fact, check-marked in the Alert Classification box, both on Sheet 1 and on l Sheet 2. Even though we do net agree that a weakness existed in this area, we will emphasize this concern in the onshift training outlined above. - _ _ - _ - _ _ _ - _ _ _ _ _ _ _ _ _ _ _ _ _ .

' . i 4- P-89023 -4- January 25. 1989 Item 14 pertains to the use of specific sector numbers versus the " general" affected areas icentified on Sheet 2 of the Fort St. Vrain emergency notification form (RERP-CR, Attachment B) Sheet 2 was developed jointly by the State of Colorado, Weld County and PSC per Planning Standard E.3 in NUREG 0654, Rev. 1, Supp. 1. These general areas correspond to a. pre-recorded emergency broadcast system in place for Fort St. Vrain. Tnerefore, we do not agree that Item 14 represer.ts a weakness. No corrective action is deemed necessary at this time, however, this concern will be discussed with State and Local authorities at an upcoming meeting. ..******...........** ........

267/8822-02: " Procedure RERP-EP CLASS, " Event and Emergency Classification Overview," is' deficient in that it lacks loss of power related EALs for the NOUE which are consistent with NUREG 0654, Appendix 1, Alert Item 7." PSC Resoonse: This item is admitted as a weakness. Emergency Procedure, EP-CLASS does lack a reference to a loss of power EAL per NUREG 0654, Appendix 1, Notification of Unusual Event, Item 7. The procedure is currently undergoing revision to incorporate this EAL and to reorganize the EAL format to present the EALs from the most severe to least severe (General Emergency presented first, down to 4-Hour Notification Events). This process is scheduled for revision completion, training and implementation by April 28, 1989. . .**.......**..

The current notification forms utili:ed at Fort St. Vrain were developed with information provided in NRC Information Notice 85-78 and in coordination with appropriate ! State and Local agencies per Planning Standard E.3 of NUREG 0654. l The NRC's Notification Worksheet identified in Information Notice 85-78 utilizes light water reactor terminology and basic event titles. Fort St. Vrain developed its notification form to correspond to appropriate HTGR terminology and events. It is agreed that the notification form does not-include a statement pertaining to whether a release is, or is not,. in progress. This statement was removed during the last revision of the form and in agreement with State and Local authorities. A meeting has been set with State and Local authorities to discuss further revisions to this form which will affect their respective plans and procedures. This meeting is scheduled for February 3, 1989. Following the meeting, any specific revisions to the notification form along with the development of a follow-up notification form will be initiated, trained on and implemented with a scheduled completion date of April 28, 1989. ........**.......**.................. 267/8822-04: "The staff at the Personnel Control Center (PCC), which is the licensee's equivalent to the OSC, did not maintain a continuous radiation exposure record of in plant teams according to checklist ' number eight of the RERP Implementing Procedure, " Activation of the Personnel Control Center." The PCC staff did not log previous exposure, verify dosimeter readings, or maintain a running record for emergency workers re-entering the plant." PSC Response: This item is admitted as a weakness. The PCC staff did not properly utilize Checklist 8 to RERP-PCC. This weakness is scheduled to be presented witnin the 1989 retraining program for PCC staff personnel. Also, all PCC Directors will be notified via company memo of the importance of assigning required duties per the implementing procedure. The memo to all directors was issued on January 17, 1989, and retraining on this issue will be ongoing througnout the 1989 RERP retraining schedule. No other corrective action is planned.
