Supplemental information for license renewal Gentlemen:
Enclosed are six (6) copies of replacement pages to be introduced into document IRT 4171-013 " Application for Renewal of Special Nuclear Materials License SNM-1405." These pages reflect changes in the make-up of the Radiation Safety Organization. Changed pages, with the exception of page number changes only, carry the date of the change as opposed to the date of the primary document.
As mentioned in the referenced letter it is expected that there will be some organizational changes taking place. At the present time we are not certain when this will occur and, since you are acting upon the license renewal, we are submitting the above mentioned material at 4
this time.
