Gentlemen: l This letter addresses the inspection of your facility at Houston, Texas conducted j by Mr. James T. Conway of this office on August 3-6, 1987, and the discussions of his findings with Mr. Daniel N. Hanna, Jr. and other members of your staff j at the conclusion of the inspection.
The purpose of the inspection was to conduct a programmatic evaluation of the implementation of the QA program in the areas of training / qualifications, {
control of purchased services, control of measuring and test equipment, audits ,
(internal / external), and reporting of defects. Areas examined during the NRC l inspection and our findings are discussed in the enclosed report. This inspec- !
tion consisted of an examination of procedures and representatives records, interviews with personnel, and observations by the inspector.
During this inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Significant items included failures to:
(1) impose QA and Part 21 requirements upon vendors, (2) have procedures /
instructions for activities affecting quality, and (3) audit / survey vendors supplying services.
The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to submit to this office within 30 days from the date of this letter a written statement containing:
(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and l (3) the dates your corrective actions and preventive measures were or will be completed. We will consider extending the response time if you can show l good cause for us to do so.

You are also requested to submit a similar written statement for each item which appears in the enclosed Notice of Nonconformance.
