APPENDIX A NOTICE OF VIOLATION I

As a result of the equipment qualification inspection conducted on. l August 25-28, 1986, and in accordance with the " General Statement of Policy for i NRC Enforcement Actions", (10 CFR 2, Appendix C) (Enforcement Policy), the j following violation was identified: l 10 C_FR 50.49(f) requires that qualification of each component be based on testing or experience with identical equipment or with similar equipment with a supporting analysis to show that the equipment to be qualified is  ;

acceptable.

Contrary to the-above, during the course of the inspection on August 27,

'1987, the inspector identified two Limitorque operators located in the reactor building at elevation 326' and 338', containing 9 cabla splices ,

whose qualification was not established, either by test, analysis or '

combination of both.

This'is a Severity Level IV, Supplement I.

Pursuant to-the provisions of 10 CFR 2.201, Power Authority of the State of New York is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or ex-planation in reply, including: (1) the corrective steps which have been taken i

and the results achieved; (2) corrective steps which will be taken to avoid I further violations; and (3) the date when full compliance will be achieved, Where good cause is shown, consideration will be given to extending this response time, j 1
