.i ENCLOSURE ADDITIONAL INFORMATION ON VAGUE EMPLOYEE CONCERNS

1. XX-85-071-003
2. XX-85-071-004 Both concerns are identical. The concerned individual (CI) is afraid that two things may not have been done correctly in the replacement of the Sequoyah high pressure feedwater heater (MFWH #1). First, the CI believes i that the nozzle should have been stress-relieved by heating during the welding process because of the thickness of the nozzles. Second, because the heater was a different diameter than the original, the CI is concerned that the piping may have been cold-sprung for fit-up during replacement.

The CI was transferred prior to the replacement and therefore, the CI cannot be sure that these concerns were addressed.

3. SQM-6-004-017 The CI overheard a discussion in which an individual asserted that certain -

work had not been done to Westinghouse Field Service Division's (WFSD) satisfaction because TVA management had hurried them. This particular concern addresses the technical issue which is not stated clearly in the file. From a transcript in the file, it appears that the CI has no more information than the following: In early 1985, WFSD rushed the installa-tion [or modification] of some tubes on the refueling floor. From con- ,

text, the NRC staff infers that the tubing was soldered using some l:

l relatively uncommon process. We also infer that, in late 1985, the tubing l

! leaked and required repair. The discussion was not entirely clear and a i transcript of the followup interview in which QTC attempted to get more l detail provided no additional information. l NOTE: This concern, SQM-6-004-017, is the technical portion of l cancelled concern SQM-6-004-013.

4. XX-85-101-002 l The CI alleges that in 1978 at SQN, an cpen drainage pipe was "T-ed" into

' a sewage line and directed sewage into the discharge canal. OTC opened an investigation and had determined that the treatment plant was for construction use and has since been torn down. However, the file does not >

indicate that the line was closed off. Also on November 5, 1985, QTC con-tacted Mr. William Pierce, TVA's Sspervisor of Environmental Compliance on this subject. There is no indice. ion in the file of any action on this allegation by QTC or TVA after Nuvember 13, 1985. Clearly this is not within NRC's regulatory purview, but as a matter of public health it ought

! to be resolved.

5. XX-85-116-005 The concern is regarding poor quality work at Sequoyah. The file does not contain specific information. identifying components or systems with quality or design problems.
6. BFM-6-002-001 This concern essentially suggests that the TVA OA/QC process has too much paperwork and procedures and that " Quality" could be better served by l

reducing and simplifying the QA/QC process and paperwork. 'No specific j deficiency is identified and the intention of the CI was categorized as " hopefully stop[ ping] a future problem."

7. SOM-6-009-001 In 1982, the CI inspected welds on a set of safety-related heat exchangers at SON and found linear indications by performing magnetic particle testing.

The CI's supervi1or had previously inspected and apparently accepted the same welds. The CI't. report was suppressed.
