This procedure replaces both the Fitness For Duty Drug Screening procedure and the On-Site Drug Screening procedure.

4.0 GENERAL INSTRUCTIONS

5.0 PROCEDURE i

5.1 Pre-Employment Testing Requirements 1 l

l' 5.1.1 All new employees and transfers in the following groups are required to submit to a pre-employment drug screening within their first 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of' employment and prior to being

o Georgia Power Company, o Project Field Engineering.

o Temporary Services.

o Construction Contractor

5.1.2 The respective Georgia Power Company, Project Field Engineering, or Contractor Management Representative is responsible for identifying those new employees and transfe,rs who require drug screening and for ensuring they report to the GPC Employment Processing Facility for processing. " Drug Screening Required" must be typed along the bottom of the employee's Vogtle identification badge 0014D Page 2

s Procedure For Drug And/Or Alcohol. Screening For All Vogtle Project Employees card. The completed card must be presented when the employee reports for drug screening.

5.1.3 "The Vogtle identification badge card must be signed by the Testing personnel at the drug screening facility, verifying the employee submitted to a drug screening prior to the issuance of the employee's card.

5.1.4 Exceptions Employees in the identified groups who will be temporarily assigned to the Vogtle project for a period of less than 30 days may not be required to submit to a drug screen. When a Vogtle identification badge is to be issued to an employee who is being exempted from the drug screening requirement, " Drug Screening Not Required" must be typed along the bottom of the employee's badge card.

5.2 Discretionary Testing Requirements Employees identified as requiring discretionary drug and/or alcohol testing according to the Vogtle Project anti-drug policy and the Management Action proced re are

5.3 Employee Consent For Drug And/Or Alcohol Analysis 5.3.1 When an employee reports to the GPC Employment Processing Facility, the l appropriate testing procedure will be explained by the Georgia Power Company Testing personnel. The employee will be asked to complete a pre-numbered Employee Consent for Drug and/or Alcohol Analysis, Exhibit 01. The employee will be asked to list all medications (prescription and over-the-counter) now being taken or

5.3.2 If an employee refuses to. submit to drug 'and/or alcohol analysis or refuses to sign the Exhibit 01 consent form, refer that person back to the appropriate Management Representative for dispositioning.

5.4 Urine Drug Analysis 5.4.1 Collection And Handling Of Urine Samples 5.4.1.1 The employee will be escorted by the Georgia Power Company (GPC) Testing

'( Personnel to the urine sample collection and handling area. The employee will be required to present positive picture _

identification to the GPC Testing Personnel along with the Exhibit 01 consent form.

.. 5.4.1.2 The employee will complete "

two pre-numbered labels (same number as on the Exhibit 01 consent form) with the employee's name, employer, date, and signature. This will be done in the presence of the GPC Testing Personnel and the Testing Personnel will initial the two labels as a witness.

5.4.1 3 The employee's urine sample of at least 80 milliliters will be collected in the {

. All Vogtle Project Employees Power Company (GPC) Testing Personnel. The GPC Testing Personnel shall measure the -

temperature of the urine -

)

with a digital thermometer. ]

If the temperature is less- '

than 96 degrees Fahrenheit, the sample shall be declared )

invalid unless the Testing Personnel can attest to the validity of it. If the temperature is greater than 98.6 degrees Fahrenheit, the employee's body temperature shall be evaluated to ensure proper relationship between ,

the two temperatures. The i temperature or signature of the Testing Personnel attesting to the sample shall be recorded on the employee's Exhibit 01 consent form.

( 5.4.1.4 In the event no female Testing Personnel are available, a female Security Officer will witness the sample collected.

l 5.4.1.5 The Testing Personnel, in l the presence of the employee, shall split the 80 milliliter urine sample into two screw-cap containers F (one of which must contain 50 milliliters) and place the caps on both l containers. The Testing Personnel, in the presence of the employee, shall attach the completed labels to the urine-sample containers and peal the containers with fragile evidence tape in a manner which seals both the cap and label to the container. The employee will then be released to the appropriate Management Representative.

All Vogtle Project Employees 5.4.1.6 The 30 milliliter (ml) sample shall be used for

- . on-site testing. The 50 ml sample shall be maintained in the refrigerator and used for, confirmation testing, if needed.

5.4.2 On-Site Testing Of Urine Samples 5.4.2.1 All urine drug testing shall be initially conducted at the on-site facility using the 30 milliliter-sample.

, Georgia Power Company Testing Personnels who have successfully completed the Syva Emit Drug Detection System Course shall conduct the testing per proper procedures of the Syva Company for the Auto Lab 6500 or QST System for Emit testing.

5.4.2.2 Testing shall be conducted for the following substances using calibrator

- concentrations as indicated:

  • Calibrator Substance Concentration Cannabinoids (THC) 75 ng/ml Phencyclidine 75 ng/ml e Cocaine Metabolite 0.3 ug/ml Opiates 0.3 ug/ml Benzodiazepines 0.3 ug/ml Barbiturates 0.3 ug/ml Amphetamines 0.3 ug/ml Methaqualone 0.3 ug/ml a ng/ml = nanograms per milliliter ug/ml = micrograms per milliliter 5.4.2.3 All-calibration, set-up, and testing documentation shall be maintained at the GPC Employment Processing Facility.

i- Procedure For Drug And/Or Alcohol Screening For All Vogtle' Project Employees 5.4.2.4 All results shall be recorded on Drug and/or Alcohol Screening Log, Exhibit 02, along with appropriate employee information. This log shall be maintained at the GPC Employment Processing Facility.

5.4.2.5 All positive results which cannot.be explained-by medications listed on the Exhibit 01 employee consent form shall be tentatively reported to the appropriate--

Management Representative immediately via telephone, or in person. The results shall be confirmed pursuant to paragraph 5.4.3. The sample being used for on-site testing shall be re-sealed with fragile

(,, evidence tape and frozen for future reference if positive results are indicated.

5.4.2.6 All negative results, and positive results which can be explained by medications listed on the Exhibit 01 employee consent form, shall ""

be' reported via Confidential Drug and/or Alcohol Screening Report, Exhibit l 03, which is to be completed L

by the Testing Personnel forwarded to the appropriate Management Representative.

A copy of this form shall be maintained at the GPC Employment Processing Facility along with the employee's consent form.

Both urine samples may be disposed of if negative results are indicated.

All Vogtle Project' Employees 5.4.3 Transfer Of Custody And Confirmation Of Positive Results 5 . 4 . 3' .1 All on-site positive results which cannot be explained by medications listed on the Exhibit 01 employee consent form shall be confirmed by the Contracted Toxicological Service.

5.4.3.2 The Georgia Power Company (GPC) Testing Personnel shall complete the appropriate information on Urine Sample Custody and Request for Confirmation of Modified Drug Screening, Exhibit 04. The GPC Testing Personnel shall have the Authorized Courier sign and date/ time the form in the

" Sample Delivered By" space. One copy of the form shall be retained. The original and two copies of the form and the respective sealed 50 milliliter urine sample shall be placed in a _

locked c~ontainer by the GPC Testing Personnel and transported to the

. Contracted Toxicological Service by the Authorized "

Courier. One key to the container shall be kept at the GPC Employment Processing Facility and the other key shall be kept at the Contracted Toxicological Service. .The Courier will not have access to the samples during transport.

5.4.3.3 Upon receipt of the locked container,.the Contracted Lab Technician (CLT) shall unlock the container, remove the sample, and document. .

All Vogtle Project Employees sealed urine sample has not been tampered with, and sign and date/ time the Exhibit 04 form in the " Sample Received By"' space. The Authorized Courier shall retain one copy of the signed form and leave the original and one copy with the CLT for reporting of confirmation testing.

5.4.3.4 If the Contracted Laboratory Technician (CLT) notes any tampering with the sealed urine sample, the CLT shall retain the sample and notify the Georgia Power Company Construction Safety Supervisor, or his designee, by telephone immediately.

5.4.3.5 The Contracted Toxicological Service shall test the urine

(. sample using the Emit methodology for the same substances at the same calibrator concentrations

- listed for on-site testing in paragraph 5.4.2.2. All

testing shall be confirmed by Gas Chromatography / Mass Spectrometry GC/MS. All results shall be recorded in the appropriate spaces on the Exhibit 04 form. The form shall be signed and dated / timed by the GC/MS Specialist and/or the Laboratory Director. The Contracted labo.ratory will notify the Georgia Power Company Employment Processing Facility by telephone when the results have been completed, and place the original Exhibit 04 form in a sealed envelope 0014D Page 9

5.4.3.6 Upon receipt of written confirmation results, the Georgia Power Company (GPC)

Testing Personnel shall notify the appropriate -

Management Representative immediately via telephone, or in person. The Testing Personne1'shall-complete an-Exhibit 03 report form and' hand deliver this, along with a copy'of Exhibits 01 and 04, to the appropriate Management Representative.

Copies of all positive test results will also be hand <

delivered to the GpC Labor Relations Coordinator to facilitate "Bar" letters on employees with positive drug / alcohol test results.

li A copy of all forms shall be maintained at the GPC Employment Processing Facility.

5.5 Breath Alcohol Analysis 5.5.1 All breath alcohol testing shall be conducted using the Intoximeter 3000

" i by Georgia Power Company Testing Personnel ~who have successfully completed the Theory and Operation of the Intoximeter 3000 course conducted by the Georgia Police Academy in  ;

conjunction with the Georgia Bureau of l Investigation. All tests shall be conducted in accordance with proper j procedure established by the Intoximeter 3000 company.

5.5.2 All calibration and testing documentation shall be maintained at  !

the GPC Employment Processing Facility.

5.5.3 All results shall be recorded in the Exhibit 02 log, along with appropriate .. l employee information. The log shall j.

5.5.4 All results shall be reported to the appropriate Management Representative immediately via telephone, or in person. The employee shall be retained at the GPC Employment Processing Facility for escort, or released, pursuant to instructions by the appropriate Management Representative.

5.5.5 The results shall then be reported via

.the Exhibit 03 report form which is to be completed by the Georgia Power Company (GPC) Testing Personnel and hand delivered to the appropriate Management Representative. Copies of all positive test results are to be hand delivered to the GPC Labor Relations Coordinator. A copy of the completed form is to be maintained at the GPC Employment Processing

(- Facility, along with the Exhibit 01 employee consent form.

- Awards will be tentative, until tne employee successfully completes the following:

(1) Drug screening test (2) Psychological test (3) Background investigation (4) Any other present or future requirement as set forth by the Nuclear Regulatory Commission or other government agencies.

- During the time between the tentative award end the final award, the successful bidder will remain at his old location, performing work in his old classification and receive pay at his old rate.

- One employee is not to be considered as held over during this time and is not eligible for payment of any expenses. .

- Once the tentative award is accepted, the employee may not witndraw his bid.

- After the employee has completed all the above requirements, his award will be final. He will receive pay at the new rate vnen he reports to his new work location.

- Employees who suct essf ully complete all the requirements for a 3cb at a nuclear plant will nave their seniority ad3usted back ,

to the date of tne tentative award.

- If an employee is unsuccessful in completing all the receire-ments for a job, he will have accumulated no seniority as a result of the tentative award, but will continue to accumulate seniority in his old classification.

- if the senior bidder is not successful in cualifying, the Ccmpany does not have to re-post the Job. Other bidders on
