SUBJECT:

INSPECTION RESULTS, LAWRENCE LIVERMORE NATIONAL LABORATORY PERSONNEL EXAMINATION OF RACEWAY SUPPORT BOLTED CONNECTIONS AT DIABLO CANYON (DN 50-275 AND 50-323)

The purpose of this memorandum is to document the results of an examination of bolted connections of raceway supports at Diablo Canyon. This effort was conducted for two reasons; first, we have received allegations related to the adequcy'of the anchor bolts which secure raceway supports to concerte (RV-83-A-033), and second, based on the feed-back I have been receiving from the Lawrence Livermore National Laboratory (contract) personnel, it appeared that adequacy of bolting of raceway supports might be a safety issue.

I requested the contract inspectors to tabulate and transmit to me the number of raceway supports examined, the number of bolts / nuts examined,-and the number and type of deficiencies observed. This information was transmitted to'me on November 8, 1983 and is current as of October 28, 1983.

Raceway Supports Examined In Detail ---- 81 Raceway Supports Partially Examined ---- estimated to be 200 to 300 l

Through Wall Anchor nuts / bolts ---- 10 to 20 examined:

One found deficient, item of non-compliance issued, licensee j

reinspected all similiar installations, 10 out of several hundred (estimated) found lose and tightened.

Other Anchor nuts / bolts ---- 1,000 to 1,500 examined:

Two were found loose, licensee tightened and documented with Minor Variation Reports (MVR)

Spring Nuts ---- 1,000 to 1,500 examined:

Based on this information, I recommend we continue to use our contract inspectors to examine raceway supports at the current level of effort.

Should the licensee's deficiency rate decrease further however, I think we should examine some other area with potential problems.
