cylinders as specified by Section 8 of the application shall be written and approved prior to receiving any cylinders.

- 13. In addition to the applicable D0T regulations, the licensee shall comply with 1
10 CFR Part 71 and 10 CFR 20.205.

- 14. Smear surveys of the valve and plug area of the cylinders shall be perfonned on a semiannual basis.

- 15. Release of equipment or materials for unrestricted use or from controlled to i
uncontrolled areas onsite shall be in accordance with the enclosed " Guidelines for Decontamination of Facilities and Equipment Prior to Release for Unrestricted i
