~ Serving The Best Location in the Nation PERRY NUCLEAR POWER PLANT NUCLEAR QUALITY ASSURANCE PROGRAM POLICY The Cleveland Electric Illuminating Company recognizes the need for a formal and comprehensive Nuclear Quality Assurance Program for the Perry Nuclear l Power Plant Unit 2 during the construction and/or indefinite period of delayed
- construction. This program is considered vital to the safety, security and well-being of the public, our customers, employees and shareholders. As agents for the ovners, we maintain ultimate responsibility for the development and implementation of a program that contributes to the safe and reliable j design, construction, extended maintenance, and operation.
It is the policy of this Company that such a program fully comply with the regulatory requirements of 10CFR50, Appendix B; ASME Section III, Article NA 4000; ASME Section XI, Article IVA.1000, as applicable, and any additional regulatory guides, codes and standards which address quality assurance requirements that are set forth in the Safety Analysis Reports for the Perry Nuclear Power Plant.
An evaluation of the philosophy, policies and general requirements disclose that the ASME and CEI Quality Assurance Programs are completely compatible in scope, purpose and intent.
The Cleveland Electric Illuminating Company has the responsibility for the accumulation and retention of all required records and documentation. In addition, CEI is committed to ASME Section III, Article NA 3230, and has contracted with Hartford Steam Boiler Inspection and Insurance Company as its authorized inspection agency. The inspection agency is granted access and freedom to the project to perform all required inspections and audits. The Cleveland Electric Illuminating Company agrees to comply with ASME Section III, Article NA 3270 by preparing, certifying and filing the required 7 N-3 Data Report. j Quality assurance is recognized as a most demanding function in terms of the '
interdisciplinary efforts that are required. Therefore, the program vill define specific individual and organizational responsibility and authority.
It vill also prescribe: procedures for compliance with regulatory requirements and establish appropriate guidelines for implementation of these procedures.
All Company and other associated personnel involved in or responsible for the quality of the Perry Nuclear Power Plant Unit 2 shall comply with this program. All Company personnel are responsible for implementation of t se j

r The Senior Vice President,. Nuclear Group, is assigned the responsibility for assuring that the Nuclear Quality Assura*ce Program fs established and implemented. He shall periodically review the overall' effectiveness of the Nuclear Quality Assurance Program and has authority to administer corrective
- action to improve its effectiveness or-to avoid potential problem areas.
The Nuclear Ouality. Assurance Department Manager is responsible for assuring the' development of the Nuclear Quality Assurance Program.' He is responsible for assuring through audit, or-other quality assurance techniques, that established systems are implemented, adequate, and are in fact, effective !
methods which will provide management with confidence that we are proceeding- ,
according to plan. He is further responsible for verifying and. documenting !
. that we'have met all regulations, commitments and other established requirements with respect to safety-related activities, systems and equipment.
4 The Nuclear Quality Assurance Department has the responsibility to identify quality' problems, initiate, recommend or provide solutions and verify implementation:of solutions. To accomplish these tasks, the Nuclear Quality
- Assurance Department is hereby given the authority to stop unsatisfactory vork or control further processing, delivery or installation of nonconforming material pending resolution of identified quality related problems.

[ CORPORATE NUCLEAR QUALITY ASSURANCE PROGRAM INTRODUCTION The President'of The Cleveland Electric Illuminating Company directed the establishment of a formal and comprehensive Nuclear Quality Assurance Program for the design, procurement, manufacturing, construction, preservation maintenance, and testing of the Perry Nuclear Power Plant. i This manual, the " Corporate Nuclear Quality Assurance Program," has been i developed to. provide a quality assurance program during construction / delayed J construction of PNPP Unit 2 which complies with the regulatory requirements of 1 10CFR50, Appendix B; ASME Section III, as applicable, Article NA 4000; and~
those regulatory guides, codes, and standards which address quality assurance requirements that are set-forth in the Safety Analysis Reports for the Perry Nuclear Power Plant.
A supplement to the Corporate Nuclear-Quality Assurance Program, and organized ';
to the 18 criteria, is the Project Administration Manual containing administrative policies and procedures applicable to all project activities.
Supplementing the Project Administration Manual to control activity-specifically related to the extended maintenance of Unit 2 equipment du.ing construction delay is the Project Maintenance Activity (PMA) program manual which addresses the coordinated responsibilities of Engineering, Quality Assurance, and the Project Work Organization. )
Subtiered to the Project Administration Manual is a series of procedure manuals which include procedures describing interdepartmental'or intercompany flow. These procedures avoid clerical details where such details are covered by intra-element instructions.
At the lowest level are instruction manuals. Such manuals are generated to y accommodate intra-element clerical details as required by procedures.
The instruction, procedure, and PMA manuals are interfaced, codified and controlled through a review process detailed in the Project Administration Manual to readily identify implementation of the Corporate Nuclear Quality Assurance Program requirements and responsibilities.
Further, The Cleveland Electric Illuminating Company has contracted Gilbert ,
Commonwealth, Incorporated, as the project's architect / engineer, and General Electric Company as the supplier of the nuclear steam supply systems. Both of these organizations are required to have acceptable quality assurance programs meeting the full intent of 10CFR50, Appendix B. This corporate manual, project policies and procedures describe the interface between CEI and its agents, consultants, contractors, and vendors. The delegated work assignments are described herein with The Cleveland Electric Illuminating Company maintaining ultimate responsibility for the project.

1 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY REQUIREMENTS 1.7. Requirements to Meet the Objective In implementing the policy of The Cleveland Electric l Illuminating Company, organizational requirements shall be.
established as described herein for the Perry Project to achieve the objectives of design, procurement, manufacture, i construction, maintenance of Unit 2 throughout the extended
'I construction delay and preoperational testing, and as delineated on the organization chart appearing at the end of this section (Attachment 1). In this revision, Attachment 1
'i ' depicts only those organizations actively involved with the preservation of Unit 2 equipment and materials during'the period of extended construction delay.
.l.2.2 Objective of the Perry Project The overall objective of the Perry Project is to construct and place in service, on schedule at optimum cost and quality, a nuclear power plant consisting of two nominal 1,200 MV electric generating units. The Perry Nuclear Power Plant (PNPP) vill utilize General Electric Company (GE) Boiling Vater Reactors (BVR6) and the Mark III containment concept. The architect / engineer is Gilbert Commonwealth, Incorporated (G/C). !

5 overall coordination of PNPP activities is the responsibility of the Cleveland Electric Illuminating Company (CEI). In this capacity, CEI has.
delegated major detail design responsibilities to both GE and G/C. The project also utilizes consultants working within the Project Organization for supplemental technical and clerical assistance.
1.3.1 GE Authorization GE'has the authority for the design, fabrication supply, licensing support, and start-up technical direction of equipment. comprising the BVR6 Nuclear Steam Supply System (NCSS) as described in Volumes I and II of the Technical Description dated. June, 1972, and design support for the Mark III~ containment concept. CE also has the authority for ~
fabrication of the initial fuel loads for unit one and two and to provide design hterface information. GE also has the authority for additional work as requested by CEI and agreed to by GE through contract amendments.
1.3.2 G/C Authorization G/C has the authority for the design of the plant including-the Mark III containment such that the GE NSSS properly interfaces with the remaining plant systems. G/C authorization also includes: licensing support, including preparation and
. administration of the safety analysis reports (FSAR) under the direction of CEI; design, including an independent design review; preparation of both equipment and construction specifications, drawings and contract packages to obtain proposals 'and evaluation of the technical aspects of equipment and construction package proposals. G/C also prepares test
{ specifications and procedures.
1.3.3 CEI Management Responsibilities Overall respc,nsibility for achievement of the Perry Project is that of the Senior Vice President, Nuclear Group. Reporting to I the Senior Vice President, Nuclear Group, is the Vice i President, Nuclear Operations Division, and the Manager of the ,
I Nuclear Quality Assurance Department. Reporting to the Vice President, Nuclear Operations Division are the Managers of the Perry Plant Operations Department, Perry Plant Technical Department, and Nuclear Engineering Department.
Administratively, the Senior Vice President, Nuclear Group, reports to the President who reports to the Chairman of the Board of Directors. Procurement support, community relations (including off-site emergency planning and public information),
and providing generel administrative services for the Perry Project is the responsibility of the Perry Services Department.
The Manager, Perry Services Department reports to the Vice President, Finance and Administrative Group, who reports to the President.

(Buyer, providing team leadership).
..The contract teams have been established to ensure overall contractor performance, within budget, schedule, and quality requirements. The contracts teams are composed of representatives from the Nuclear Engineering Department, (Contracts Administrator providing team leadership, and Responsible Engineer) and Nuclear Quality Assurance Department (Mechanical / Civil Quality Engineer).

A. Nuclear Construction Services Section Nuclear Construction Services Section (NCSS) is responsible for the sequencing and administration of contracted construction activities, for completion and coordination of vork, for monitoring the contractors, progress in assembling documentation, and for providing support on an as-required basis to the Nuclear Operations Division in the preservation and maintenance of PNPP Unit 2.
This Section is composed of experienced construction and engineering personnel, who vill function as an integral part of the CEI, NED.
NCSS is responsible for administration of l contracted construction activities including the monitoring of the field activities of contractor personnel which have been procured for special assignment, limited scope work and for providing related administrative services.
B. The Mechanical Design Section The Mechanical Design Section is respon-sible for I&C, piping, and mechanical equipment along with the corresponding installation specification activities, and for defining storage / maintenance require-ments for the PMA program.
C. The Electrical Design Section The Electrical Design Section is respon- '
sible for electrical equipment and instal-lation specification activities, on-site conduit detailing effort, support of the ERIS installation and for defining storage / maintenance requirements for the PhA program.

D. The Engineering Project Support Section !
l The Engineering Project Support Section is i responsible for updating and maintaining I as-built drawings, for equipment qualifi-cation, for maintenance of the 0-List, and for procurement of equipment (spare parts). I l
II. Nuclear Quality Assurance Department Independent of other department is the Nuclear Quality Assurance Department (NOAD). This department has the authority and independence to plan and direct those activities affecting the overall quality program. ,
NOAD is assigned sufficient authority and i organizational freedom to identify quality problems; to initiate, recommend,_or provide solutions; and to prevent or control further processing, delivery, installation, or utilization of nonconforming items until properly resolved.
The Department consists of: Maintenance and Modification Quality Section; Operational ,
Quality Section; and Procurement and 1 Administration Quality Section.
The Quality Assurance Division of G/C has been l contracted by NOAD to provide personnel and quality assurance services in: evaluating prospective vendors; preparation of Quality Assurance specification requirements; revieving engineering specification; reviewing proposals; and providing assistance in the quality engineering and quality control functions of the NOAD. ]
The Quality Assurance Department of Raymond Kaiser Engineers (RKE) supplies personnel to ,
NOAD in areas such as: surveillance inspection activities; quality engineering; and technical and clerical support.

records establishing inspection require-ments, providing quality support for main -
tenance and preservation of Unit 2, reviewing and approving nonconformance reports, and performing surveil- .j lance / inspections and for maintaining the l quality assurance program descriptions in l the CNOAP and FSAR Section 17. I i
B. Operational Quality Section The Operational Quality Section is composed of the Nondestructive Examination Unit, !
Operational Support and Program Unit and Quality Audit Unit.
In support of the Unit 2 preservation, the Operational Quality Section is responsible for providing nondestructive examination support for the preservice and inservice inspection program.
The Quality Audit Unit is assigned responsibility for quality assurance audit functions, for the review and approval of test procedures of the Unit 2 test organization, for implementing nonconformance control procedures commencing with jurisdictional transfer ,
from contractors to PNPP, and for ensuring that NR's initiated subsequent to jurisdictional transfer are reviewed and approved. The Quality Audit Unit trains and qualifies audit personnel; plans and coordinates the internal audit program; conducts audits; tracks and follows up on audit findings; and performs trend analyses and special evaluations including the tL.1'yDN

C. Procurement and Administration Quality Section The Procurement-and Administration Quality Section is composed of the Procurement' Ouality Unit and the Administration and Records Unit. The. Procurement and Administration Quality Section is responsible for implementing quality record requirements, tracking status of quality-related documents, coordinating agent services, coordinating reviews of procedural documents, administering department training and qualification programs, and preparing administrative reports. Procurement and Administration Quality Section is also responsible for- i coordinating agent services,. coordinating design, procurement, and manufacturing quality activities, such as review of purchase orders and contracts, and vendor QA Manuals participation in supplier preavard meetings,' conducting external (supplier) audits and performing receipt inspections of owner-supplied materials and '
equipment. Additionally, the section is responsible for inspections of the CEI varehouse compound.

1.1 POLICY The Cleveland Electric Illuminating Company's Corporate Nuclear Quality Assurance Program (CNOAP) for the Perry Nuclear Power Plant Unit 2 satisfies the requirements of <10CFR50, Appendix B; ANSI N45.2; and Regulatory Guide 1.28). Implementation of this program is mandatory for all CEI organizations involved in the design, procurement, manufacturing, construction, maintenance, inspection or testing of safety-related systems, structures or components. Upon acceptance of these systems, structures, and components by the Perry Plant Operations / Perry Plant Technical Department for start-up and operations, the operational quality assurance program shall be in effect.
The Corporate Nuclear Quality Assurance Program encompasses all equipment, systems and structures, including nuclear fuel, which have been categorized as safety-related as delineated by the Safety Analysis Report (SAR), and Fire Protection Systems as defined by the Fire Protection Evaluation Report. The level of Quality Assurance activity applied to equipment in these classes vill depend upon the safety class, equipment complexity, intended function, and other quality, administrative and engineering considerations, i It shall be the responsibility of each department manager to ensure that all responsible project personnel are cognizant of the mandatory requirements of this manual and of the procedures which implement the program. The Manager, Nuclear Quality Assurance Department (N04D) shall also be responsible for confirming that responsible CEI departments, agents, consultants, contractors and vendors comply with the applicable requirements of this program.
CEI retains the responsibility fot the development, approval, application, administration, and control of the total Quality Assurance Program for the Perry Nuclear Power Plant Unit 2. The program provides ;
for accomplishing activities affecting safety under suitably controlled conditions and in accordance with the program elements of procedures, instructions, specifications, and drawings.

1.2 THE CEI OUALITY ASSURANCE PROGRAM REQUIREMENTS The Corporate Nuclear Quality Assurance Program requires:

The objective of these requirements is to provide a high level of 'l confidence to ensure that the items or services provided satisfy ]
safety-related design requirements and the project applicable Nuclear Regulatory Commission regulations.
In order to ensure effective implementation of the program, the fc11oving provisions have been established:
- a. A CEI management organization committed to quality, with responsibilities and authorities clearly established and documented.
- b. Qualified consultants and agents to design and assist in the i construction and testing of the Perry Nuclear Power Plant. ]
- c. Competent company technical personnel to develop and administer the necessary procedures for designing, purchasing, manufacturing, .
constructing, examining, and testing plant systems, structures and l components and for monitoring program compliance.
- d. A Quality Assurance organization, with sufficient organizational freedom and authority to inspect, audit, evaluate, and initiate corrective actions on activities and items affecting the quality of safety related systems, structures and components, and to report any deficiencies found.
- e. Contractors' and vendors' adherence to their contractual scope of vork, including PNPP approved procedures affecting Unit 2 l activities.

- f. A suitable work environment which includes management access, appropriate equipment, and adequate indoctrination and training to perform safety-related activities.
1.3 QUALITY PROGRAM DOCUMENTATION i This Corporate Nuclear Quality Assurance Program Manual is the primary '
corporate document describing the Nuclear Quality As.surance Program for the construction and maintenance phases of the Perry Nuclear Power Plant Unit 2. Each of the eighteen cri*.tria of 10CFR50, Appendix B, and the responsibilities for the corresponding safety-related activities are addressed in the eighteen sections which comprise this Manual.

Page 3 of 5 The requirements of this program shall be implemented in accordance with d^ tailed procedure and instruction manuals, which are related through the hierarchy of documents illustrated in Pigure 1.
All procedures and instructions shall be approved and established, with training accomplished, prior to the start of the activities being controlled. Issuance, distribution, and revisions shall be controlled to preclude the use of obsolete documents.
Reference indexes demonstrating the Corporate Nuclear Quality Assurance Program's address with respect to the guidelines provided by Regulatory Guides and American National Standards Institute (ANSI) are presented in (Appendix I>.
1.4 MANAGEMENT ASSESSMENT Management assessment of program effectiveness shall be through regular and documented reporting channels on a month by month basis, and through a Quality Assurance Quarterly Evaluation.
An independent assessment of the program effectiveness shall be conducted annually.
In addition, the Nuclear Safety Review Committee (NSRC) has been designated to conduct regular review and evaluation of the QA Program for the Perry Nuclear Power Plant (PNPP) Unit 2. The committee shall advise the Senior Vice President - Nuclear Group on the adequacy of scope, implementation and effectivene.ss of the PNPP QA Program, and on CEI QA policy matters as they relate to PNPP.
The NSRC shall have an approved charter addressing membership, meeting frequency, authority and duties.
1.5 RESOLUTION OF PROGRAM IMPASSES Disputes arising from the interpretation of the Corporate Nuclear Quality Assurance Program should be resolved at the lowest possible level.
The hierarchy of each project department provides for interf aces from department specialists through various levels of supervision. This organizational consistency should provide ample opportunities for problem ,
resolution through simple escalation.
Those conflicts which cannot be resolved at lover lo els shall be referred to the Manager, NOAD. The Manager, NQAD shall attempt to resolve the problem with the manager of the other concerned departtent, or escalate to the executive level, using the advice of the Nuclear Safety Review Committee Es appropriate.

l 1.6 INDOCTRINATION AND TRAINING )
s 1 The Perry Training Section is responsible for the development of an overall project training program, and shall plan, develop, schedule, conduct and document training program activities in coordination with supervisors of individuals to be trained. It is a Supervisory responsibility to ensure that personnel performing quality-related activities are suitably trained and qualified to perform their work.
The Nuclear Quality Assurance Department, through its audit program, shall provide the assurance that training is meaningful, adequate for the tasks to be performed, consistent, and in accordance with the following project regulatory requirements:
- a. Inspection and test personnel shall be qualified and certified to the requirements of < ANSI N45.2.6 as endorsed by Regulatory Guide 1.58).
- b. Nondestructive Testing personnel shall be qualified and certified to <SNT-TC-1A>. ,
- c. Auditors shall be qualified and certified to the requirements of
< ANSI N45.2.23 as endorsed by Regulatory Guide 1.146).
1.7 CONTROL OF THE CORPORATE NUCLEAR OUALITY ASSURANCE MANUAL This manual was developed under the direction of the Manager, NOAD. It has been reviewed and approved by The Cleveland Electric Illuminating Company's executive officers.
Distribution of'this manual shall be controlled by the Document, Procedures, and Services Section. All recipients of controlled manuals !
are responsible for the maintenance of their assigned manual. Proposed revisions shall be directed in writing to the Document, Procedures, and Services Section. All proposed revistons must be reviewed and concurred with by the. Document, Procedures, and Services Section prior to submittal for review and approval by the Manager, NOAD.
1.7.1 NRC Review of 0A Program Changes All changes to the Corporate Nuclear Quality Assurance Program l Manual that reduce commitments in the OA program description '
previously accepted by the NRC must be submitted for NRC approval before they are implemented per 10CFR50.54(a).
Approval is considered granted upon receipt of a letter to that effect from the reviewing office of the NRC, or after 60 days from the date of submittal, whichever occurs first.
Changes to the CNOAP Manual that do not reduce ecmmitments may be made without prior NRC approval. Any such changes must t;e submitted to the NRC Region III office for information within 90 deys from the date of implementation.

1.1 POLICY l The design and design review of the Perry Nuclear Power Plant Unit 2 (PNPP) is provided by Gilbert Commonwealth, Incorporated (G/C) as the Architect / Engineer and General Electric as the Nuclear Steam Supply System (NSSS) supplier. Both organizations have been delegated total design authority and are required to have written procedures which meet the requirements of <10CFR50, Appendix B, and ANSI N45.2.11) in accordance with the CEI Nuclear Quality Assurance Program.
CEI performs a design coordination function consisting of selected j reviews and a design control monitoring program. This function is the responsibility of the Nuclear Engineering Department, and the Nuclear Quality Assurance Department which are to perform these activities in accordance with approved, detailed procedures. These procedures shall assure that:
- a. Design activities are conducted in a planned and systematic manner.
- b. Perry Safety Analysis Report requirements have been appropriately addressed in design documents.
- c. Design requirements can be controlled and inspected and/or tested to specified acceptance criteria.
1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY DESIGN CONTROL REQUIREMENTS In addition to project management techniques, CEI design control objectives are met by maintaining various reviev and approval functions over Safety Analysis Reports, 0A Programs, Balance of Plant Design Specifications, and selected dravings.
1.2.1 Safety Analysis Reports The development, maintenance and control of the PNPP Safety Analysis Report (SAR) is a CEI, GE, and G/C licensing effort.
G/C, GE, and CEI are all responsible for input of SAR informa-tion while, G/C is responsible for production. CEI is respon- .
sible for approval and issue authorization. There are three distinct stages of SAR preparation and maintenance. They are prior to Construction Permit, Construction Permit until Final Safety Analysis Report (FSAR) docketing, and FSAR docketing until Operating License.

Prior to a Construction Permit, the Preliminary Safety Analysis Report (PSAR) is maintained via an amendment process.
' Amendments may be generated in response to NRC Requests for Information. Upon receipt of a Request for Information, the CEI Licensing Engineer requests G/C to initiate an amendment.
\ Draft Amendments are distributed to responsible CEI departments for review. After comment resolution with G/C, the amendment ;
is published, filed with the NRC, and distributed. i i
After receipt of a Construction Permit, the PSAR is maintained by SAR Deviations. As final, detail design evolves, variations from PSAR requirements may develop. These shall be documented on SAR Deviations and receive the same project review and approval as the PSAR. G/C shall prepare the SAR Deviation by their own initiations or CEI notification. Upon G/C approval, the SAR Deviation is forwarded to CEI for a reviev and approval by responsible departments and for a determination by the Licensing and Compliance Section (if approved) to issue as an SAR Deviation or for later inclusion in the FSAR or reviev vith the NRC for their approve before acceptance as an SAR Deviation.
The Final Safety Analysis Report (FSAR) is based upon the PSAR and SAR Deviations. Development, review, issue, and amendment to the FSAR is a~ accomplished via the PNPP Change Request process. FSAR changes are proposed by an identifying Section, evaluated per provisions of 10CFR50.59, approved for submittal at the General Supervisory level, and reviewed by knowledgeable organizations, including Licensing and Compliance Section, to ensure legal, regulatory and licensing requirements remain fulfilled.
1.2.2 Specifications Balance of Plant safety-related specifications generated by G/C as the Architect / Engineer shall be reviewed and approved by CEI Nuclear Engineering Department and Nuclear 0A Department. i Preliminary copies shall be controlled by correspondence, final copies and revisions shall be controlled (released to Perry Services Department) by documented Engineering 0A Releases.
Specification development, issuance, and affectivity shall be tracked by a status system. .
Changes to specifications, including engineering design change requests, shall be controlled in a similar, approved manner.
G/C shall also prepare test specifications for the balance of plant systems. These documents vill delineate test objectives, references, prerequisites (for tests), initial conditions, !
tests to be performed, and acceptance criteria for developing Preoperational and Acceptance Test procedures. These test l specifications shall be developed on a system basis, reviewed YWWh.

CEI Nuclear Engineering Department receives controlled copies of General Electric (NSSS) specifications for information, interface considerations and reference for receipt, !
installation, testing and operational activities. GE shall also prepare test specifications for NSSS Systems which vill be utilized for the preparatica of testing procedures. !
1.2.3 Dravings Design information for Balance of Plant.(B0P) included in such ;
selected drawings as flow diagrams, one-line electrical diagrams, plant layout drawings, and some detailed design drawings are reviewed by CEI Nuclear Engineering Department for overall conformance to SAR requirements and CEI operating and engineering preferences.
These Engineering reviews shall be conducted and documented in accordance with written procedural requirements. They are 4 performed in addition to the independent reviews for plant criteria and design adequacy prepared by the originating organization (G/C or GE).
Balance of Plant drawings, used in lieu of specifications, shall be reviewed and approved by Nuclear Engineering Department and Nuclear Quality Assurance Department in accordance with approved procedures.

Their systems of cross-discipline and inter-company reviews of design information is complemented by CEI Nuclear Engineering Department in the role of a project management group. Engineering reviews of specifications and selected drawings (including some vendor drawings) provide additional assurance to the control systems and procedures established by ,
G/C and GE.
1.2.5 Reportable Deficiencies All deficiencies found in design and construction of PNPP, which, were they to remain uncorrected, could adversely affect the safe operation of Unit 2 at any time throughout the expected life, shall be reported to the Nuclear Regulatory Commission. Current Federal Regulations delineate two categories of reportable noncompliance and two system requirements. These requirements are defined under

Rev.1 Effective 1/16 /87 Page 1 of 3 PROCUREMENT ~ DOCUMENT CONTROL 1 J 2, 1.1 POLICY i-Procurement'of material, equipment and services for safety class systems, structures and components on the Perry Nuclear Power Plant (PNPP) Unit 2 shall be performed in'accordance with written policier and procedures and instructions. These shall establish methods for preparation, review, approval, and control of procurement documents and shall provide measures to comply with requirements of <10CFR50, Appendix B).
1.2 THE CLEVELAND ELECTRIC ILLUMINATING PROCUREMENT DOCUMENT CONTROL REQUIREMENTS CEI Nuclear Group and Perry Services Department shall establish detailed procedures and instructions to control the procurement of safety-related equipment, material and services. Heasures shall be established for the review, approval and release of procurement documents, and subsequent proposals, revisions or change orders. These reviews shall assure the inclusion of the applicable technical, quality, and commercial requirements in procurement documents prior to their use.
Procedures shall also be. established to monitor the procurement document control programs of the delegated purchasing responsibilities of GE (NSSS), Gilbert Commonwealth, Incorporated (G/C), Contractors and Vendors.
Professional' services of consultants and agents shall be procured by Perry Procurement and Records Management Section upon submittal of a requisition by the organization requesting the service. Professional service agreements shall be submitted to Procurement and Administration s Quality Section for determination of applicable Quality Assurance program requirements. If Quality Assurance program requirements have been determined to apply to the original agreement, changes to the agreement shall also be submitted to the Procurement and Administration Quality Section.
The measures established for the control of procurement documents shall include the following requirements: ,

- c. Qualified personnel shall review and concur with the adequacy of quality requirements to determine that the9 are correctly stated, inspectable, and controllable; that there are adequate acceptance I
criteria; and that procurement documents have been processed in accordance with established requirements.
- d. Review of procurement documents shall be documented to evidence their approval prior to their release.
- e. Vendor and contractor quality programs shall be reviewed and approved prior to initiation of the activities affected by their programs. '
- f. Procurement measures shall require that changes and revisions to procurement documents be subject tc the same control requirements as the original documents.
These aforementioned program requirements shall be implemented in a manner that assures procurement documents:
- a. Specify applicable <10CFR50, Appendix B> and codes or standards requirements which must be complied with, and described in, the vendor's or contractor's Quality Assurance Program.
- b. Specify design basis technical requirements, including applicable regulatory requirements, material and components identification requirements, drawings, specifications, codes and industrial standards, test and inspection requirements, and special process instructions be identified or referenced in procurement documents.
- c. Identify the documentation to be prepared, maintained, and submitted for reviev, approval and record information as applicable.
- d. Ider.tify those records which vendors or contractors shall retain, maintain, and control; and those records which the vendors or contractors shall deliver prior to use or installation of the item.
- e. Include rights of access to vendors or contractors and their subtier vendor and contractor facilities and records for source inspection and audits.

Corporate Nuclear Quality Assurance Program 0500 Rev. 8 Effective 9 / 16 / 87 Page M f 4 INSTRUCTIONS, PROCEDURES AND DRA7INGS 1.1 Policy i The Cleveland Electric Illuminating Company requires that activities affecting quality of safety-class structures, systems, and components be prescribed in and accomplished by documented instructions, procedures, and drawings. Participants in the project shall be responsible for the preparation and use of appropriate documents which describe the control of their quality-related activities. These documents shall be developed as appropriate for the quality-related activity and its importance to safety. They shall include appropriate quantitative and qualitative acceptance criteria sufficient to verify that the activity has been satisfactorily accomplished. Before work is begun on a quality-related activity, the controlling documents shall be approved and personnel who perform the work shall be trained in their requirements.
CEI shall comply with the requirements for the establishment, issuance and. control of instructions, procedures, and drawings in accordance with
<10CFR50, Appendix B> and < ANSI N45.2>.
1.2 The Cleveland Electric Illuminating Company Instructions, Procedures and Drawings Requirements The CEI Quality Assurance Program shall be delineated in policies, procedures and instructions developed by each department, and processed through the Document, Procedures,.and Services Section. Drawings as design documents are controlled as delineated in < Corporate Nuclear Quality Assurance Program 0600), " Document Control".
1.2.1 Procedures and Instructions Established'by Project Departments Procedures and instructions shall be established by each Project department to implement the following requirements:
- a. Activities affecting quality shall be described by an approved instruction, procedure, or policy.
- b. Instructions, procedures, or policies shall be developed, reviewed, approved and issued in accordance with this Corporate Nuclear Quality Assurance Program manual and the detailed requirements specified by approved procedures.
- c. The Nuclear Quality Assurance Department (NQAD) shall review instructions and shall review and approve those procedures and policies which delineate the methods of {
complying with the requirements of 10CFR50, Manual and gd j the criteria specified by <10CFR50, Appendix B>.

- d. Each instruction, precedure, or policy shall describe the sequence of action to be accomplished and shall define the responsibilities and authorities of Project personnel performing the activity.
- e. Appropriate quantitative and qualitative acceptance criteria shall provide measures for determining the satisfactory accomplishment of important activities.
- f. Interfaces with other Company elements or other agent activities shall be clearly described.
- g. Instructions, procedures and policies shall be reviewed and approved, and identified project personnel shall be trained in their content, prior to performing quality-related work.
- h. Distribution of instructions, procedures and policies shall be adequately controlled to preclude the use of obsolete documents and shall be sufficiently complete to assure availability to responsible personnel.
- i. Vendor Instruction Manuals, or portions thereof, used for inspection, test or work activities affecting quality of safety-class structures, systems, or components shall be reviewed, approved, and issued by the Nuclear Engineering Department and issued by Information Control Unit (ICU) prior to use,
- j. Revisions to instructions, procedures, and policies shall be subject to program controls equivalent to those imposed on original issue documents.
- k. Documents which supplement the procedure system such as Special Project Plans and Record Expediting Action Plans (REAP's) shall conform to the requirements of this section.
l 1.2.2 Procedures and Instructions of Consultants. 1.3 Responsibilities Each project department shall be responsible for the preparation and implementation of instructions and procedures in accordance with the requirements specified in this Manual.

Document, Procedures, and Services Section shall ensure the development of detailed preparation, approval, and maintenance procedures l to control the development of CEI instructions, procedures, and policies.
The General Supervising Engineer, Procurement and Administration Quality Section, is responsible for ensuring the QA review and approval of vendor QA manuals and procedures; and for establishing vendor audit and manufacturing surveillance programs; including those of GE NSSS and l nuclear fuel.
The General Supervising Engineer, Maintenance and Modification Quality -
i Section, is responsible for ensuring the OA review and approval of instructions, procedures, and the Project Maintenance Activity Manual.
The Section shall also be responsible for verifying, through inspection / surveillance, the adequacy and implementation of the Project Maintenance Activity for PNPP Unit 2 during the delayed construction phase. CONTROL OF PURCHASED MATERIAL, EQUIPMENT AND SERVICES 1.1 POLICY The procurement controls of material, equipment, and services for the construction, extended maintenance and reoperation testing phases shall be performed in accordance with written policies and procedures. Quality Assurance measures shall' apply to the procurement of materials including spare parts, replacement parts, off-the-shelf-items, and consumables.
The procurement controls for spare and replacement parts shall be equivalent to those applies to the original procurement. Procurement of spare and replacement parts to requirements' differing from those of the original procurement vill be permitted provided a detailed design-evaluation is completed prior to procurement.
Appropriate measures shall be included to evaluate procurement sources, monitor the activities of consultants, vendors and contractors, and confirm that purchased items conform with procurement document requirements. The programs of all participants shall be in accordance t with the CEI Specifications, this CEI Nuclear Quality Assurance Program .
and <10CFR50, Appendix B>. l The general and specific requirements for. the Quah[y Assurance Program of all vendors and contractors, including their subvendors and subcontractors supplying safety-related material, equipment, or services are delineated by procurement documents. The procurement documents impose Quality Assurance Program requirements that are commensurate with the degree of complexity and the importance.to safety of the items and services being performed.
1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY'S CONTROL OF PURCHASED EQUIPMENT, MATERIALS, AND SERVICES REQUIREMENTS The control of procured safety-related material, equipment, and services shall be provided by the CEI Nuclear Group and Perry Services Department (PSD) in accordance with established procedures. Quality Assurance procedures provide measures to ensure that CEI purchased material, equipment, and services conform to the specified requirements of procurement documents. These Quality Assurance measures consist of ,
various procurement, manufacturing, receiving, inspection and installation control procedures which shall be established to implement the following requirements.
1.2.1 Procurement Assurance Procedures Shall Require:
- a. Procurement documents and specifications are reviewed for technical requirements by Nuclear Engineering Department (NED).

applicable to the items and services requested.

For each vendor included in this program, a detailed audit and/or surveillance plan shall be established and implemented in accordance with approved surveillance and audit procedures. Scope of coverage and frequency shall be determined by the criticality of the furnished items and the evaluated results of vendor qualification analysis including preavard surveys and procedure preparation performance. Revisions to surveillance and audit plans shall be made as warranted by vendor performance.

These measures shall , assure that incorrect or nonconforming items are identified and controlled in order to prevent their inadvertent installation or use at , the Perry Nuclear Power Plant Unit 2. Where required, by design documents, the system established shall provide traceability of components from the receipt of material through fabrication and testing. Verification shall include objective evidence of inspections and tests that product identification and control is maintained at various stages of manufacture, installation or erection, and preoperational test. Identification requirements shall be specified in the applicable design and procurement documents, dravings, procedures, and instructions used for the Perry Nuclear Pover Plant Unit 2. 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY IDENTIFICATION AND CONTROL OF MATERIALS, PARTS, AND COMPONENTS REQUIREMENTS Heasures shall be established to implement the following requirements:

1.3 Responsibilities The Senior Vice President, Nuclear Group, is responsible for ensuring the establishment of measures which assure that identification requirements are incorporated into procurement documents specifications, drawings, procedures, and instructions. These measures shall include controls to prohibit or restrict the use of potentially hazardous / detrimental materials or instruments containing potentially hazardous / detrimental material that could cause contamination or affect the integrity of personnel, fuel, zirconium, carbon steel, stainless steel, or any surface within the entire Nuclear Steam Supply System. The General Supervising Engineer, Procurement and Administration Quality Section, is responsible for the review of documents regarding procurement to ensure the inclusion of identification requirements. Inspection, surveillance, and audits shall be conducted of vendors to assure compliance with identification and traceability requirements. The General Supervising Engineer, Procurement and Administration Quality Section, is responsible for the review of vendor's OA program manual to assure measures have been established for identification and control of items. Also ! responsible for verifying through inspections that items stored in CEI's I varehouse compound are appropriately identified and controlled. i l The General Supervising Engineer, Maintenance and Modification Quality l Section, is responsible for conducting surveillance / inspections of items issued to the field to assure compliance with identification and traceability requirements. Velding, casting, i electrochemical machining, explosive forming, heat treatment, special ! cleaning, nondestructive examination and other processes which require unusual care or close control shall be performed in accordance with appropriate written procedures. Procedures for special processes vill be ' established to meet the requirements of applicable codes and standards vhere available, or to meet the requirements of special process specifications which vill be produced for the Perry Nuclear Power Plant (PNPP). All procedures shall be in accordance with The Cleveland Electric Illuminating Company (CEI) Nuclear Quality Assurance Program and

1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY CONTROL OF SPECIAL PROCESSES , CEI procurement documents shall require contractors and vendors to ! establish measures that ensure control of special processes. Procurement I specifications shall require that special processes vill be accomplished l under controlled conditions and, as appropriate to the scope of work, I that appropriate provisions in the following codes, standards, recommended guides, and engineering requirements are met. 1

Control of special processes shall be ensured by Nuclear Quality Assurance Department through surveillance / inspection and audit of manufacturing, maintenance and construction activities in order to assure compliance with procedures, specifications, codes, standards and guides.

Qualification and certification of inspectors and operators who perform special process functions are verified through audits of certification records. Special process procedures shall include provisions for the following requirements:

- b. Special processes shall be performed with qualified personnel in accordance with written process sheets, or equivalent, with provisions for recording evidence of verification,
- c. Filing and maintenance provisions for qualification records of procedures, equipment, and personnel associated with special processes shall be established and kept current at the manufacturing facility for vendors or the construction site for contractors.
During the Pre-operational Testing Phase, the control of Preservice i Examination and Test activities vill be in accordance with the provisions of the CNQAP manual as applicable to contractor and CEI personnel. 1.3 RESPONSIBILITIES The Manager, Nuclear Engineering Department (NED) and Nuclear Quality Assurance Department (NOAD), are responsible for the establishment of ; special process engineering and quality requirements in procurement t documents and the establishment of measures to ensure the control of special processes. They are also responsible for the establishment and approval of CEI special process procedures, and, where applicable, those procedures necessary to ensure that repairs and replacements performed by CEI and contractor personnel are conducted in accordance with the ASME Boiler and Pressure Vessel Code. The General Supervising Engineers (GSE's), NED, shall be responsible for revieving and approving Project Organization's special process procedurer and for coordinating this reviev vith Maintenance and Modification Quality and Operational Quality Section personnel. The GSE's, Maintenance and Modification Quality and Operational Quality Sections, shall be responsible for implementing special process control through surveillance / inspection prograns to ensure compliance. The GSE, Procurement and Administration Section, shall be responsible for reviewing and approving procurement documents to ensure the inclusion of the applicable special process requirements. The CEI Nuclear Quality Assurance Department (NOAD) either directly or through its agents, shall ensure the implementation of the Perry Nuclear Power Plant (PNPP) Unit 2 inspection program. This shall be accomplished by reviewing and approving the inspection programs of each vork-organization, by performing inspection and/or surveillance during manufacturing, construction, maintenance, preoperational testing, or preservice examination and testing, and by conducting a comprehensive ! audit program. Personnel responsible for implementing the Perry Nuclear Power Plant Unit 2 inspection program shall meet the qualification requirements of < ANSI N45.2.6>. 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY INSPECTION REQUIREMENTS Inspection of components, systems, structures and their safety-related activities for the PNPP shall be conducted in accordance with the approved program by individuals who are organizationally independent from the personnel performing the work. Comprehensive inspection programs consisting of plans, procedures, instructions, and checklists shall be established for manufacturing, l construction, maintenance, preoperational testing, and preservice examination and testing activities by the Operational Quality Section l (005), Maintenance and Modification Quality Section (MMOS), and the Procurement and Administration Quality Section (PA0S). Design and procurement documents shall require that vendors and contractors establish appropriate inspection programs for their work and provide access for the NOAD inspection / surveillance program. I 1.2.1 Manufacturing Inspection i l The Procurement and Administration Quality Section shall j establish, within the Manufacturing Assurance Program, a -) manufacturing surveillance plan with CEI vendors and General l Electric as the Nuclear Steam System Supplier (NSSS). I Inspection point coverage shall be based upon specification requirements, manufacturing processes, complexity and criticality of the manufacturing requirements and , manufacturer's recommendations. Adjustments to frequency of inspection monitoring shall be made as varranted by period evaluation of the manufacturer's inspection program. These manufacturing hold and vitness points, and manufacturing surveillance of vendors and audits of General Electric NSSS, shall ensure control of manufacturing inspection.

This program activity shall be conducted in accordance with approved inspection planning documents developed for each vork organization, and consists of a number of techniques ranging from reinspection to verification of work organization's activities. The frequency and extent of surveillance / inspection shall be based on evaluations or previous surveillance / inspections, nonconformances, and audit trend results. The Procurement and Administration Quality Section shall implement a receiving inspection function. This program consists of established inspection plans for various types of receiving inspection requirements and checklists. 1.2.3 Inspection Programs Inspection Programs shall include but not be limited to the following requirements:
- a. Inspection programs shall be conducted in accordance with approved, detailed procedures, instructions and checklists which shall provide for:
- 1. The identification of the characteristics and activities to be inspected.
- 2. The identification of the individuals or groups responsible for performing the inspection.
- 3. The acceptance and rejection criteria for the inspection.
- 4. Descriptive instructions for performing the inspection.
- 5. The documentation requirements to evidence l manufacturing, construction, maintenance, inspection or test operations.
- 6. The inspection result evaluations and reporting requirements.
- 7. The inspector qualification requirements.
- 8. The specifying of necessary measuring and test equipment and the appropriate accuracy requirements. .

Testing of safety class and selected non-safety class structures, systems, and components shall be conducted in accordance with approved test procedures which incorporate the requirements and acceptance limits contained in design documents. The test program consists of vendor product tusts, contractor construction verification tests, maintenance tests, pre-operational tests, ASME Section XI preservice tests, and selected Initial Check-Out and Run-In (IC&R) and acceptance tests which require implementation of the Quality Assurance Program. These testing activities shall comply with the requirements of design and procurement documents and <10CFR50, Appendix B>. 1.2 THE CEI COMPANY'S TEST CONTROL REQUIREMENTS The control measures established for test programs shall include but not be limited to the following requirements:
- a. Equipment or system deficiencies identified during the course of testing programs shall be documented by nonconformance reports and/or approved test deficiency reporting procedures.
- b. Detailed and approved equipment turnover control methods.
- c. Adequate and appropriate test equipment.
- d. Completeness of item to be tested, and the use of calibrated and controlled test instrumentation,
- e. Certified test personnel.
- f. Suitable testing environmental conditions and prerequisites have been met, including, as appropriate, test equipment, cleanliness, temperature and, if necessary, humidity.

- g. Detailed and approved testing procedures and instructions, hold or vitness points for inspection by the Quality Assurance representative of the owner or contractor, (as required), and acceptance and rejection criteria.
- h. Provisions for appropriate data collection, review and record storage.

- j. Requirements and acceptance limits are contained in applicable i design and procurement documents, with procedures for ensuring that modifications, repairs, and replacements are tested in accordance with the original design and test requirements or by approved alternatives.
Specifications shall be prepared by responsible design organizations j delineating test requirements. Detailed test control procedures shall be ] ' established by Vendors, contractors, Unit 2 test organization and the Perry Plant Technical Department for all safety-related test activities, including preservice baseline testing required by ASME Section XI. Procurement documents identify and/or establish product tests to ensure that the manufacturer's product vill perform satisfactorily. Product tests include pump tests, valve leak rate tests, shop hydros, and continuity tests. These product tests are conducted by the vendor and monitored by the Procurement and Administration Quality Section. Construction and maintenance verification tests are static tests conducted by work organizations to ensure that components of a given system are installed and maintained in accordance with specifications, drawings, and manufacturing instruction manuals. These verification tests include hydrostatic, electrical continuity, and pneumatic tests.
, These tests are conducted at the construction site and the results reviewed under the direction of the Nuclear Construction Services Section and shall be monitored by the Maintenance and Modification Quality Section.
The Unit 2 test organization shall be responsible for implementing the e pre-operational and acceptance test program. This phase of testing iacludes the initial checkout and run-in tests such as stroking valves and bumping motors to verify proper installation and operation of individual components in a system. After initial checkout and run-in, pre-operational and acceptance tests shall be conducted to demonstrate the capability of systems and components to meet performance and design j criteria. Corporate Nuclear Ouslity Assuranca Program 1200 Rev. 6 Effective 9 / 16 / 87 Page G f 3 CONTROL OF MEASURING AND TEST EQUIPMENT 1.1 POLICY Measures shall be established to ensure that tools, gauges, instruments, and other measuring and testing devices used in activities affecting safety class structures, systems, and components be properly controlled,. calibrated, and adjusted at specified periods to maintain accuracy within specified limits. 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY CONTROL OF MEASURING AND TEST EQUIPMENT REQUIREMENTS The CEI procurement documents shall require suppliers furnishing safety class equipment, materials and services-to maintain a program for the control of measuring and test equipment. The programs shall be implemented through the use of written procedures, and compliance vill be verified by the Nuclear Quality Assurance Department (FOAD) program review and approval, and through inspections, surveillance and audits. CEI Nuclear Group and agent personnel performing surveillance,. inspections, and tests shall establish detailed procedures for the control of their measuring and test equipment. Qualified firms may be employed to provide calibration services from their facilities. It shall be the responsibility of these firms to maintain and submit all documentation requested and to provide for control of their testing equipment in accordance with procurement documents that comply with this section of the Nuclear Quality Assurance Program. Requirements for each control program shall include inspection and verification of accuracy upon receipt of equipment, identification of all gauges and instruments, calibration and scheduled recall for calibration when due, and traceability to an accepted Standard. Procedures shall be established to implement the following requirements:
- a. To establish the calibration technique and frequency, maintenance, and control of all measuring and test equipment which is used in the
- measurement, inspection, and monitoring of safety-related components, systems, and structures (instruments, tools, gauges, fixtures, reference and transfer standards, and nondestructive test equipment).
- b. Identifying measuring and test equipment traceable to the calibration test data.
- c. Indicating status of calibration for each piece of measuring and test equipment, indicated by use of labels or tags indicating the date of the next calibration check.

- d. Establishing calibration frequency for measuring and test equipment based on required accuracy, purpose, degree of usage, stability characteristics, and/or any other condition which may affect the measurement.
- e. Methods for determining the validity of previous measurements when the measuring and test equipment is found to be out of calibration.
Inspections or tests are repeated on items determined to be suspect. Such determinations to be documented in suitable form and reported immediately to responsible CEI personnel. t-
- f. Calibration standards shall have an accuracy range and stability which are adequate to verify that the equipment being calibrated is within the required tolerance. The tolerance of calibration standards shall be better than the tolerance of lover level calibration standards being calibrated.
Calibration of measuring and test equipment used for measuring, gauging, testing, inspection or control to determine compliance with design, specifications or other technical requirements shall be calibrated against standards that have an accuracy of at least four times the required accuracy of the equipment being calibrated. When this is not possible, standards shall have an accuracy that assures the equipment being calibrated vill be within required tolerance and that the basis of acceptance is documented and authorized by the supervisor of the calibrating organization.
- g. A status of all measuring and test equipment under the calibration program to be maintained.
- h. Utilizing references and transfer standards traceable to nationally recognized standards. Where national standards do not exist, provisions to be established to document the basis for the calibration.
- i. Methods of packaging, handling and shipping (where applicable) measuring and test equipment such that the accuracy is not impaired.
1.3 RESPONSIBILITIES The Manager, Perry Plant Operations Department, Nuclear Operations Division, is responsible for ensuring the establishment of procedures for the control of PNPP-owned measuring and test equipment. The Manager, NOAD, is responsible for ensuring the establishment of NOAD measuring and test equipment control procedures, the review and approval of the Nuclear Operations Division measuring or test equipment control procedures, and establishment of audit, inspection, and surveillance programs to verify compliance.

Status of safety and applicable nonsafety class structures, systems, and components shall be controlled in accordance with approved procedures. These procedures shall include the use of appropriate tags, markings, , lists, logs, diagrams, or other suitable means, to assure that required inspections and tests are satisfactorily completed, to prevent inadvertent bypassing of required inspections and tests, and to prevent inadvertent operations. 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY INSPECTION, TEST, AND OPERATING STATUS REQUIREMENTS Procurement documents shall require that contractors, vendors, and agents provide adequate inspection, test, and operating status programs commensurate with their scope of vork. The Nuclear Quality Assurance Department (NOAD) shall administer the project status indicating measures and shall accomplish this activity through procedure reviews, surveillance, inspections, and audits. 1.2.1 In addition to these administrative measures, the CEI program for inspection, test, and operating status shall include detailed procedure requirements to maintain the status oft

Disposition of I nonconformances is made by the organization that established the I governing requirements or by other qualified individuals or committees authorized by Cleveland Electric Illuminating Company (CEI). 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY NONCONFORMING MATERIALS, PARTS, OR COMPONENTS REQUIREMENTS The Nuclear Quality Assurance Department shall monitor the nonconformance control programs of each participant in the Corporate Nuclear Quality Assurance Program, periodically analyze Nonconformance Reports to observe the quality trend, and report to CEI management for review. Such analysis vill be based upon severity, the number, and frequencies of nonconformances, the causes of the nonconformances and the timeliness and adequacy of the reporting and resolution of nonconformances. CEI procurement documents shall require that contractors and vendors of safety-related materials, parts, or components have procedures controlling nonconforming conditions. The Nuclear Quality Assurance Department shall review and approve the , nonconformance control measures of vendors, contractors and agents, and shall verify implementation by surveillance, inspections and audits. Procedures shall be established to implement the following requirements:
- a. Identifying, documenting, segregating, reviewing, dispositioning, and notifying the affected organizations of nonconforming parts, materials, components, or services.
- b. Documenting the nonconformance, including at least the following: i
- 1. Description of the nonconformance. 1 l

- 2. Disposition of the nonconformance (repair rework, use-as-is or y scrap), all as defined by (ANSI N45.2;10).
- 3. Evaluation of the impact of identified conditions adverse to quality on the design and operability-of the plant.
- 4. Measures.to assure that the cause of the condition is determined and corrective action taken to preclude repetition.
- 5. Inspection requirements and any associated acceptance criteria that differ from those originally specified.
- 6. Signatures approving the disposition and verifying its implementation.
- c. Identifying and notifying, through established document control measures, individuals or groups to whom responsibility and authority has been delegated for approving the disposition of the nonconforming item.
- d. Identifying nonconforming items as discrepant, and where possible, segregating them from acceptable items until the discrepancy is J resolved.
- e. Prior to initiation of a pre-operational test on a safety-related structure, system or component, all nonconformances shall be evaluated for impact on further testing.
f ., Verifying acceptability of reworked or repaired material, parts, components, systems, and structures by reinspecting and retesting the item to the original criteria and acceptance levels, except where equivalent alternatives have been specified and approved. All inspection, testing, rework, and repairs shall be by approved procedures and the results shall be documented. Replacement items shall be- subject to the same, or better, inspection and test controls as the original items.
- g. Submitting Nonconformance Reports dispositioned "USE.-AS-IS" or
" REPAIR" for assessment and approval by the responsible design l authority and Quality Assurance prior to effecting the disposition. l The Responsible Organization shall assure GE and/or G/C concurrence
- l vhen the NR affects the GE and/or G/C scope of design or supply.
The indication of GE and/or G/C concurrence shall either be included on the NR form (e.g.: countersign line 11) or attached to it.
- h. CEI furnished items which are dispositioned " SCRAP" require CEI approval in order to initiate procurement of a replacement. l

He is also responsible for ensuring that nonconformance trends are evaluated and reported to project and CEI management. The General Supervising Engineer, Operational Quality Section, shall implement nonconformance control procedures commencing with jurisdictional transfer from construction contractors to the Project Organization. Also responsible for ensuring that Nonconformance Reports initiated af ter jurisdictional transfer from construction contractors are reviewed.for significant deficiency deportability in accordance with
<10CFR50.55(e)> and <10CFR2D.
The General Supervising Engineer, Procurement and Administration Quality Section, shall also provide for the review and approval of procurement documents and specifications for purchased equipment and services other than construction, installation or erection work to assure that nonconformance control requirements are specified. He provides for the review of vendor Quality Assurance Manuals to assure the inclusion of nonconformance control requirements. He is responsible for verifying the implementation of nonconformance control systems at vendor facilities through surveillance and audit techniques and for documenting the results. He is responsible for ensuring that Nonconformance Reports initiated by vendors are reviewed for significant deficiencies in . accordance vith <10CFR50.55(e)> and <10CFR2D. j The General Supervising Engineer, Maintenance and Modification Quality Section, shall implement nonconformance control procedures for surveillance / inspection of work by on-site work organizations during the maintenance and construction phase. He is responsible for verifying and documenting the implementation of nonconformance systems through surveillance / inspection of the work organization's activities. He is responsible for ensuring that Nonconformance Reports initiated by contractors, and those initiated by the Project Organization during the construction and maintenance phase, are reviewed for significant deficiencies in accordance with <10CFR50.55(e)> and <10CFR2D. The General Supervising Engineer, Procurement and Administration Quality Section, is responsible for logging and tracking Nonconformance Reports, and for reporting the current status of open Nonconformance Reports to l CEI Management. l The General Supervising Engineers, Nuclear Engineering Department, are responsible for providing engineering resolution of nonconformances. 'l.1 POLICY-Measures shall be established for the orderly preparation, accumulation, filing, turnover, storage, and maintenance of quality' assurance records by all project participants.
Each of these~ individual record systems is to be established based on project administrative procedures or as specified in procurement documents. These systems shall be described and controlled by approved written procedures and instructions, adequately implemented, and verified l by Nuclear Quality Assurance Department (NOAD) through surveillance and audits, 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY OUALITY ASSURANCE RECORDS REQUIREMENTS 1.2.1 The accumulation of Quality Records during the design,
].. procurement, manufacturing, construction, maintenance and preoperational testing phases of the Perry Nuclear Power Plant Unit 2 is accomplished by two systems. .l
- a. Upon completion of each contractor and vendor organization's safety-related activity, their QA Records shall be accumulated and shipped to the Perry Nuclear Power Plant Unit 2 construction record file in accordance with procedures. Shipment procedure and schedule for each organization shall be as documented on an approved transfer plan. Detailed procedures shall be prepared for the receipt, handling, reviev, approval and storage of vendor and contractor quality records. j 1
- b. The accumulation of quality records generated by CEI, consultants, and agents shall be accomplished through approved procedures and transfer plans. Detailed procedures for their receipt, handling, review, and filing shall be established and approved prior to commencing transfer to permanent storage.
1.2.2 Measures shall be established for the ultimate collection and permanent filing of all quality records. These measures shall provide for the permanent file indexing and retrieval system, the microfilming of the records, and the record storage i facility. j l

Corporate Nuclear Quality Assurance Program 1800 Rev. 7 Effective 9 /--16 / 87 Page Iof 3 AUDITS l.2 POLICY A comprehensive system of planned and systematic audits shall be established and folloved to ensure that quality requirements are implemented, adequate, and effective and to verify compliance to the Corporate Nuclear Quality Assurance Program for Unit 2. The audit program shall encompass the design, procurement, manufacturing, construction, maintenance, installation, and testing of safety-related , materials, equipment, and~ structures by all Perry Nuclear Power l Plant (PNPP) Unit 2 participants. The Nuclear Quality Assurance 1 Department (NOAD) shall establish measures for implementing the overall audit program. NOAD may delegate some of the audit functions to 0A agents or establish programs for sharing of audit information, such as through Coordinating Agency for Supplier Evaluation (CASE). Such j delegated functions shall be audited by NOAD, and all audit reports shall I be reviewed to evaluate audit program effectiveness. PNPP audit programs l shall be implemented in accordance with written procedures which include: responsibilities assigned, audit frequencies, reporting criteria and levels of management to receive and assess audit findings as I required by <10CFR50, Appendix B). j l 1.2 THE CLEVELAND ELECTRIC ILLUMINATING COMPANY AUDIT REQUIREMENTS 1 The NOAD maintains the responsibility for the overall audit program and monitors all audit activity to ascertain program effectiveness. DISPOSITION - Resolution of the immediate actions required of a nonconformance item or procedure (e.g.: repair, rework, use-as-is, scrap). INSTALLATION - The' placing of parts, components, and equipment into their functional position and joining to the overall plant system. INTENT, LETTER OF - A Letter of Intent is a letter sent to the contractor or vendor indicating the owners' intention to ender into a contract with the contractor. ITEMS OF INTEREST REPORT - This is a weekly memorandum from each department manager to the vice president of the department's group. The memorandum consists of single paragraph summaries of significant department activities, occurrences and accomplishments. MANUFACTURER - An organization which provides finished parts or com-ponents for use. NEW SPEC - A computerized system which in part monitors specification status. ) NONCONFORMANCE (NONCONFORMING) - A deficiency in characteristic, documentation, or procedure which renders the quality of an item unacceptable or indeterminate. Examples of nonconformance include: physical defects, test failures, incorrect or inadequate documen-tation of deviation from prescribed processing, inspection or test procedures. NUCLEAR QUALITY ASSURANCE DEPARTMENT (NQAD) - That portion of the CEI organization which directs and controls the QA program. NUCLEAR QUALITY ASSURANCE DEPARTMENT MANAGER (NQADM) - The CEI staff member in charge of the NQAD who reports directly to the Vice President - Nuclear Group. l CORPORATI NUCLEAR QUALITY ASSURANCE PROGRAM (CNQAP) - The document which establishes the program of planned and systematic activities which ; provide assurance of quality during design, procurement, con-struction and test. OWNER - The Cleveland Electric Illuminating Company, agent for Duquesne Light Company, Ohio Edison Company, Pennsylvania Power Company, - Toledo Edison Company, and The Cleveland Electric Illuminating Company, owners of undivided interests in the Perry Nuclear Power Plant Project, Units 1 and 2, collectively referred to as the Central Area Power Coordinating Organization (CAPCO). PERFORMANCE ANALYSIS REPORT (PAR) - This is a formal monthly report from each department to the vice president of the department's group. This report details departmental activity and progress on project assignments; summarizes manpower and timekeeping information and general department progress; delineates costs, and identifies special problems.
