Gentlemen:

Thank you for your letter dated June 10, 1987, in response to the violation

(454/87016-01(DRS)) which we brought to your attention in our letter dated

May 11, 1987. Your response requested that we review the severity level

assigned to the violation based on your belief that it was a documentation

problem and that our report stated it was of minor safety significance.

The violation involved both the documentation and the performance of a surveillance

procedure required to satisfy the Technical Specifications. The problems

identified in the violation were not a single incident, but the pervasive failure

to properly perform the surveillance during the core reload activities.

Procedure 1B0S 0.1-6, Temporary Change No. 87-1-283, and Procedure BOP 199-T6,

were not adhered to; and the personnel performing the procedures were not

knowledgeable of the proper methods to complete the surveillances. Based on the

inspection and review of the available documentation, it was concluded that the

Technical Specification limits were not exceeded. The severity level of the

violation was based on the failure to properly perform the surveillance, on the

personnel confusion which existed durir.g the performance of the procedures,'and

the resultant inadequacy of the documentation for the surveillance. We do not

believe that the violation was a simple documentation problem, and therefore,

, the violation was designated a Severity Level IV. This classification is

consistent with the description of a Severity Level IV violation in Supplement I

of the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

Based on the above, we thereby confirm our previous classification of the

violation as a Severity Level IV.
