ENCLOSURE 1 NOTICE OF VIOLATION Georgia Power Company Docket No. 50-424 Vogtle 1 License No. NPF-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on May 23 -
June 19, 1987, violations of NRC requirements were identified.
The violations i
involved multiple examples of failure to follow procedures.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions",
a 10 CFR Part 2, Appendix C, the violations are listed below.
Technical Specification 6.7 la. requires that written procedures be established, implemented and maintained covering activities delineated in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, February 1978.

Subsequently, control banks shall be inserted at least 500 PCM.

Contrary to the above on June 6,1987, OP No. 12003-1 was not i
properly implemented in that the unit was taken critical with the actual rod height greater than 500 PCM below the ECC height and the control banks were not inserted as required, This resulted in too high a startup rate (2.2 decades per minute) which caused the source range Hi Flux Bistable to trip on high flux level and a subsequent reactor trip during startup.

Administrative Procedure (AP) No. 00054-C " Rules for Performing Procedures" step 4.1.5 requires that if finding the procedure to be incorrect, stopping and notifying his/her immediate suaervisor, and step 4.1.7 recuires that revising procedures in accorcance with AP No. 00051-C, ;' Procedure Review and Approval" or AP No. 00052-C, Temporary Changes", when found to be incorrect prior to resuming work.
Contrary to the above, on June 13, AP No. 00054-C was not groperly implemented in that while performing surveillance 14400-1 Control Room Emergency Ventilation Actuation Logic Test" on the A train the operator did not stop per step 4.1.5 when the B train was actuated and the procedure did not address or allow for restoration and the shift supervisor did not revise the procedure per AP No. 00052-C when notified that the procedure steps for resetting the radiation monitor was incorrect.

Operationsi Procedure No. 11213-1 "Backflushable. Filter System Alignment for Startup' and Normal Operation" requires valve 1-1418-U4-071,. Demineralized Water to. Backflushable Filter System to-be closed.
l Contrary to the above, on May 18 and 19, OP 11213-1 was not
'l implemented in that upon removal of a clearance' valve 1-1418-U4-071 was not maintained in its required-position.
This mispositioning-resulted in contamination of the demineralized water system.
1 Items 1, 2, and 3 above, singularly-and collectively constitute a violation for_ failure to follow procedures.
This is a Severity Level. IV Violation (Supplement I).
Pursuant to 'the provisions of 10 CFR 2.201, Georgia Power Company is hereby required to submit to this '0ffice within 30 days of the date of this Notice a written statement or explanation in rep (ly) including for each violation:
(1) admission or denial of. the violation; 2
the reason for the ' violation. if -
admitted; (3); the corrective steps which have been taken and the results

achieved; (4) the corrective steps-which will be taken to avoid 'further violations; and (5) the date when full compliance 'will be achieved.
Where-cause is shown,_ consideration will be given to extending the-response time. good l
