RESPONSE TO INSPECTION REPORT 87-022 Nuclear Regulatory Commission Inspection' Report 87-022, dated November 25, 1987 transmitted one violation requiring a written response within 30 days.

Consumers Power Company's response is provided herein.

l Violation - (50-155/87-022-01)

As a result of the inspection conducted on August 31 through November 17, 1987, and in accordance with 10 CFR Part 2, Appendix C, " General Statement of Policy and Procedure for NRC Enforcement Actions (1985)," the following

'1 violation was identified:

10 CFR Part 50, Appendix B, Criterion XVI states that measures shall be established to assure that conditions adverse to quality are promptly identified and corrected.

l Contrary to the above, as of September 1987, the licensee had failed to-correct the administrative weaknesses identified during a March 1987 QA surveillance of the inservice testing program..Further, the licensee failed to followup on the identified weaknesses, such that violations of l

10 CFR 50.55(a)(g) were not identified.

Discussion Consumers Power had identified weaknesses in the Inservice Testing Program in l

March, 1987, during a QA Surveillance. Corrective action for the identified deficiencies were reviewed and assigned a due date of December, 1987. The reason for this due date was twofold. Firet, the surveillance was. completed during the 1987 Refueling Outage. At that time, the total attention of the ISI-Engineer was required to review and approve all testing.results and 1

assembly of the Inservice Inspection' and IGSCC data for submittal to the NRC.

Second, approximately 95% of ISI related testing activities occur.during i

Refueling Outages. Since management review did not conclude that any deficiencies required immediate attention, the December, 1987 date was felt appropriate'to l

ensure all items would be addressed prior'to the 1988 Refueling Outage when the next segment of ISI related testing activities will be conducted.-

Response to Inspection Report 87-022 l

December 28, 1987 Corrective Actions Taken Procedure changes are being developed to address the items identified in the I

inspection report as follows:

1.

T30-22 will be modified to address the concern in Section 2 of the report.

This procedure will be issued by January 30, 1988 and a copy will be sent l

to Region III for review.

2.

Test procedure TSD-01 will be modified to revise acceptance criteria in accordance with ASME Section XI 1977 S78 by January 30, 1988.

(Item 3.h) 3.

All test procedures will be revised by February 28, 1988 to ensure all the l

valves in the IST program with remote position indication are verified for proper. operation. (Item 3.f) i 4.

Upper stroke time limits will be established for all IWV valves in accord-ance with ASME XI 1977 S78 prior to 1988 Refueling Outage. (Item 3.e)

Relief requests for pump flow and bearing temperature will be developed and submitted to address Item (3.b).

This action will be completed prior l

to the 1988 Refueling Outage.

6.

Instructions have been provided to the Shift Supervisors with regard to trending requirements of valve timing after maintenance (Item 3.d).

Long term actions will also include revisions to maintenance procedures to require recording of stroke times and routing.through the ISI coordinator.

l This action will be completed through the biannual review cycle of the l

procedures.

Corrective Actions To Avoid Recurrence Administrative Procedures have been drafted, reviewed, and issued describing the duties and responsibilities of individuals in ensuring compliance to the ASME Code Section XI 1977 S78 edition. During development of these procedures, a review of code requirements and implementation was completed. Relief requests will be generated as necessary.
