NRC SPECIAL INSPECTION REPORT 50-331/98006(DRS) AND

Dear Mr. Franz:
' On March 20,1998, the NRC compieted a special inspection at your Duane Amold Energy
Center. The enclosed report presents the results of that inspection.
On March 13 and 16,1998, the reactor manual control system was removed from service to
perform modifications. This special inspection consisted of a review of the circumstances -
surrounding the removal of this system from service, a review of selected procedures and
records, and interviews with the personnel involved with removing the system from service.
Areas inspected are identified in the enclosed report.

The . inspection revealed that operators and engineers took the piimary means of reactivity
control out-of-service for modifications for approximately 27 hours3.125e-4 days <br />0.0075 hours <br />4.464286e-5 weeks <br />1.02735e-5 months <br /> without a thorough review of
the implications of taking that system out-of-service during power operations. Further, they
were not timely in restoration of that system to service even though operators would have had
to rely on a safety-related reactor trip function to insert control rods if a reactivity excursion
occurred during installation of the modification. The Region til staff is concemed with the
practice of removing manual rod control from service at power and has forwarded this matter to

the Office of Nuclear Reactor Regulation for review.
Based on the results of this inspection two vio'ations of NRC requirements were identified and
l
are cited in the enclosed Notice of Violation (Notice). The first violation involved the failure to
maintain an accurate status of the reactor manual control system in the operating and
operations shift stgervisor logs. The second violation involved the failure to perform adequate
post modification testing to verify operability of all components affected by the reactor manual
control system modifications Please note that you are required to respond to this letter and
should follow the instructions specified in the enclosed Notice when preparin
