SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-010/95008:

50-237/95008: 50-249/95008)

Dear Mr. Joyce:

Enclosed are the results of our inspection conducted by M. Leach, and others

of this office, and by C. Settles of the Illinois Department of Nuclear

l Safety, on April 25 through June 12, 1995. The inspectors reviewed activities

l authorized for your Dresden Nuclear Station, Units 1. 2 and 3. At the

conclusion of the inspection, the inspectors discussed their findings with

members of your staff. .

The areas examined during the inspection are identified in the report. Within

,

'

these areas. the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress. The purpose of the inspection was to determine

whether activities authorized by your licenses were conducted safely and in

accordance with NRC requirements.

Overall, we found that the conduct of activities during this period was

satisfactory. There were two issues of concern during this period. The first

was the failure of plant staff to adhere to management expectations. Examples \ k

of this were the failure to take manual action to secure a reactor water

cleanup pump before it' tripped on low suction pressure, failure to follow a

surveillance procedure, and the routine use of human out-of-services for

maintenance. The second was the inability to take effective corrective

actions. Local leak rate test failures from the last Unit 3 refueling outage l

were not reported in accordance with 10 CFR 50.73. A violation was issued in

Inspection Report 94014 for a similar problem. I

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Based on the results of this inspection, certain of your activities appeared

to be'in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because it reflects a

deficiency _in the application of your corrective actions programs to prevent

repetitive problems.

You are required to respond to this letter and should follow the instrEctions

specified in the enclosed Notice when preparing your response. The responses

directed by this letter and the enclosed Notice are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980. Pub. L. No. 96.511.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice." a copy of

this letter and its enclosures 'will be placed in the NRC Public Document Room.
