INSTRUCTIONS FOR USE OF CK-MP3-06-00, SHEETS 3 AND 4 1.
The ORG Verifier shall fill out a copy of CK-MP3 06-00 Sheet 3 for each ICAVP system requirement.
2.
This copy of CK-MP3-06-00 Sheet 3, shall be used as the first page of the package for the entire set of checklists and other attachments for the system requirement being evaluated.
3.
During the review of each ICAVP system requirement, the ORG Verifier shall only use the checklists necessary to document the evaluation.
4.
- The checklists completed for each system requirement shall be indicated by an X in the left column of Sheet 3.
5.
The ORG Verifier shall accumulate the total number of sheets used for each checklist and use this accumulated total as the basis for the page numbering on each checklist set.
6.
The total number of pages used for each checklist shall be entered on the copy of CK-MP3-06-00 Sheet 3 for the system requirement being evaluated.
7.
Use Sheet 4 to document all additional attachments.
8.
The total number of pages for the entire set of all checklists for the system requirement being evaluated shall be entered on CK-MP3-06-00 Sheet 3.
9.
CK-MP3-00-01, CK-MP3-06-12, and any other checklists used for ARG and PRG evaluations shall npqt be included in the above system review packages. ARG and PRG items shall be evaluated and dispositioned according to their respective instructions.
